Causative agents of urinary tracts infection in children with obstructive uropathy

Summary:

167 children with different variants of congenital obstructions of urinary tracts were examined; the causative agents of urinary tracts infections were analyzed in all the cases. The changes of the cytokines levels were determined in those patients which had the more often found causative agents in the analyses. It was mentioned that of the total patients with obstructive uropathy (OU) the fifth part in the absence of bacteriuria had a pathogen in biopsy material (BM) of urinary tracts. In fourth part of cases microorganisms that were detected in BM and urine did not matched. The changes of cytokines levels in urine did not have any species-specific dependence on causative agent

Summary:

CII arch fractures are divided into groups according not to anatomical localization, but to the instability degree resulted from the trauma. Effendi et al classification became greatly widespread. |t is based on the same stability criteria, but it is simpler, intelligible and gives the plan of treatment. All the fractures according to this classification are divided into three types: the first one consists in stable fractures in which even in functional images there is no any rough deformation at the level of CII segment; the second one consists in damages with significant angular (more than 11°) or shift (more than 3,5 mm) deformations, that indirectly indicates damage of soft tissue structures, and first of all diskoligamentous complex. The services of the authors of the present classification are also that they were the first who had defined fracture-dislocation of CII arch, attributed it to the third type. Choice of surgical procedure in unstable CII-III vertebral fractures is ventral spondylosyndesis of CII-III carried out by submaxillary approach after the preliminary closed elimination of deformation of upper cervical part of spine. The differentiated approach to the implant choice with an allowance for morphometric data allows to receive adequate decompression of nervous - vascular structures and reliable stabilization of the damaged segment

Summary:

Research objective is to estimate stressful remodelling features of bone tissue according to the densitometry data and to the level of biochemical markers of bone resorption and formation in total hip replacement (THR).
Bone tissue mineral density (BTMD), condition of calcium-phosphoric metabolism and biochemical markers of bone formation (osteocalcin and bone isoenzyme of alkaline phosphatase) and resorption (С-terminal bodypeptide of the I type collagen) have been determined in 52 patients with coxarthrosis of ll-lll stages with marked joint dysfunction before and after THR. The control group included 24 donors. The data were considered to be reliable when the probability index was р<0,05.
The reliable (р<0,05) change of BTMD was determined only in 3-6 months after the operation, whereas the change of biochemical markers of remodeling had already been done after 1,5-3 months, allowing to define the group of patients with obvious negative bone balance: strong predominance of resorption processes without compensation of the subsequent adequate osteogenesis, that subsequently could lead to significant bone tissue deficiency in the area adjacent to the endoprosthesis. Changes of indices of calcium-phosphoric metabolism were not certain during the investigation term.
ln conclusion it is to state that biochemical markers of remodeling in comparison with BTMD allow to estimate objectively features of adaptive bone tissue remodeling after THR in earlier periods and to define group of patients with sharp intensification of metabolism and obvious negative bone balance

In the work comparison of two methods of pertrochanteric fracture fixation has been carried out: external one, with the help of the transosseous device and immersed one, by the dynamic femoral system. The investigation of the process of dislocation of fragments has been done on biophantomes. The dynamic compression in the external fixation device increases stability of the biomechanical system «bone-fixator» in 3-4 times. The use of single-stage fracture compression with dynamic femoral system allows to create stable fixation of fragments, adequate for early mobilization of patients
