PECULIARITIES OF LAPAROSCOPIC ADHESIOLYSIS BY PERITONEAL COMMISSURES IN ABDOMINAL CAVITY

The investigation is dedicated to surgical treatment of patients with peritoneal process of abdominal cavity. Authors elaborated the complex clinical classification of peritoneal process and its clinical manifestations in the form of peritoneal commissures, taking into account principal demands of practical surgery, as well as therapeutic-diagnostic algorithm of surgeon’s actions in the abdominal commissures. According to clinical cases it was demonstrated that laparoscopic adhesiolysis as compared with the traditional open method allows to reduce complications considerably and to avoid the development of lethal outcomes.
SURGICAL PROPHYLAXIS OF ISCHEMIC INSULTS IN PATIENTS WITH ATHEROSCLEROTIC IMPAIRMENT OF CAROTIDS

The results of prophylaxis of acute impairment of a cerebral circulation with the help of carotid endarterectomy (CEAE) are considered. There are examined 324 patients with cerebrovascular pathology 132 of them have transferred CEAEand 156 received only conservative therapy. The results of Wyears observation of operated and non operated patients show, that in patients, who were operated, in mostcases the marked signs of impairment of cerebral circulation in ipsilateral hemisphere were not developed. The results of observation over group of patients who were not operated coincide with the average data according to which the patients who have suffered an ischemic insult, have the riskof development of the subsequent insult at a level of 5-9% in a year, and approximately 25-45% of them have a repeated insult during 5 years period after the first insult. The best clinical effect had CEAE, performed in patients with transitional ischemic attacks. CEAE is the main preventive measure in treatment of patients with an atherosclerotic stenosis of carotids. The analysis of the results of the research testifies to the stable and long-term effect of operation.
FEATURES OF COGNITIVE DISTURBANCES AT THE ARTERIAL HYPERTENSIA

The questions of diagnostics and classification of cognitive disturbances in 112 patients with hypertensive encephalopathy are considered. The diagnosis of cognitive frustration was based on results of expanded neuropsychological examination with a qualitative and quantitative estimation of the received results. 56 % of the examined patients had the moderate cognitive disturbance syndrome. Neurodinamical-disregulation and primary-dismnestical types have been revealed. Taking into account, that opportunities of treatment and rehabilitation of patients with dementia noware rather limited, the study of predementical forms of cognitive disturbances when therapeutic actions are considered to be the most effective and of great importance.
THE DYNAMICS OF 0,1 HZ-COMPONENT OF HEART RATE VARIABILITY AT PATIENTS WITH ARTERIAL HYPERTENSION AT TREATMENT BY FOSINOPRIL AND ATENOLOL

Aim: to study dynamics of the 0,1Hz-componentofthe heart rate variability(HRV) spectrum in patients with arterial hypertension taking fosinopril and atenolol. 49 patients with hypertension were enrolled (age - 46+/-7 years). Fosinopril and atenolol were administered consequently. The power of 0,1 Hz-component of the HRV spectrum was assessed during a passive orthostatic test. Results: In patients taking fosinopril and atenolol comparable decrease of blood pressure was observed. Atenolol caused reliable decrease of a heart rate during orthostatic test. The power dynamics of 0,1Hz-componentwas comparable in patients taking fosinopril and atenolol. Conclusion: fosinopril and atenolol influenced similar on 0,1-Hz mechanisms of vegetative heart control system.
THE CLINICAL USE OF TRF - TOPOGRAPHY IN DIAGNOSTICS OF INFLAMMATION OF LOWER RESPIRATORY TRACT

The prospective research with some elements of double-blind study has been done to investigate accuracy of our overheated resonance radio-emission in identification of some serious inflammations of internals of a thorax. A new method of studies - trance-resonant functional topography- has been used.
The authors made an investigation with the help of the “Trance-resonant functional topograph (TRF- topograph)” apparatus in 250 patients (in 120 patients with pneumonia and in 50 patientwith acute bronchitis) and in 80 healthy persons.
It was found that patient had a rise of the resonant- waves reaction as above the region of a pathological process (inflammation) and total the resonant- waves reaction in region of examination.
After treatment in many patients there was found elevation of the resonant-waves response in healthy persons.
Some new diagnostical criteria for identification and localization of inflammatory chest disease were presented and some problems of diagnostics ofinflammation of lower respiratory tract (pneumonia, acute bronchitis).
SYSTOLIC BLOOD PRESSURE: BIOPHYSICAL, AGE AND GENDER FEATURES

The article is devoted to the study of systolic blood pressure features in the population of large industrial center. On the basis of measurement of blood pressure in adult population of three city regions there were revealed the following age-related changes in systolic blood pressure in men: three periods of rise and two periods of stabilization. The principal difference in women is the absence of the periods of rise and stabilization, lower level of blood pressure in young and middle aged people and authentically higher level of blood pressure - in elderly women. The increase of haemodynamical loading for one year of life in men of the middle (2 times) and elder ages (3 times) was revealed, that was not typical forwomen.
AMLODIPINI IN THE MANAGEMENT OF HYPERTENSIVE PATIENTS WITH GOUT

The aim of the present work was a detailed study of questions connected with the peculiarities ofcircadian blood pressure profile, efficiency of amlodipine in patients with goutand arterial hypertension. We used 24-hour blood pressure monitoring before and after 3-months treatment. Patients with gout showed the disturbances of circadian blood pressure profile (Non-dipper). Prolonged calcium antagonists are the best drugs for the hypertensive patients with gout.
SOLAR ACTIVITY AND PECULIARITIES OF THE RHEOLOGICAL PROPERTIES OF THE BLOOD IN PATIENTS WITH ANGINA PECTORIS

The peculiarities of the rheological properties of the blood are studied in patients with non-stable angina pectohs in different perbds of solar activity. It has been established, that in high solar activity hemorheological disorders are characterized by increase of blood viscosity and compensatory decrease oferythrocyte aggregation and increase of their deformation. In low solar activity less perceptible hyperviscositywas accompanied by significant increase oferythrocyte aggregation and decrease of their deformity capacity.
MULTICOMPONENT SOLUTION FOR LOCAL ANAESTHESIA IN OPERATIONS ON EXTREMITIES

The analysis of the results of local anaesthesia (LA) in 89 patients aged 18 to 68 years with trauma and diseases of extremities has been carried out. The efficiency of perineural injected multicomponent solution of clonidine added to conventional mixture oflidocaine and phentanyl has been assessed.
The multicomponent method has significantly prolonged the duration of local anaesthesia of 1% lidocaine solution providing reliable anaesthesia of plexus and peripheral nerves alongside the sufficient regional myoplegia and prolonged postoperative anesthetization. When there is no need of deep myoplegia Hallows to lower the concentration of lidocaine solution twofold, maintaining its efficiency, and to realize local anaesthesia of patients with high risk.

We’ve studied the efficacy of using siccine acid nitrates in clinical course of experimental myocardial infarction and pancreatitis. It was revealed thatanimals with myocardial injury and hypodinamia, siccine acid nitrates put obstacles in the way of increasing activity in the process of peroxide oxidation lipids, and cause moderate activation ofanti-oxidant protection.
