Summary:

In recent decades, in the sphere of minimally invasive approach to preparing hard dental tissues, adhesive dentistry has made significant progress, revolutionizing many aspects of restorative dentistry. The attitude towards cavity preparation has changed due to adhesive systems as there is no necessary to prepare cavities with additional retention points to ensure mechanical adhesion with the filling material. Thus, the introduction of adhesive systems contributes to the preservation of more non-demineralized enamel and dentin in the preparation of hard dental tissues in accordance with the principle of "biological expediency". To achieve a better connection with the hard tissues of the tooth, a number of adhesive systems has been developed. The complexity of achieving a better connection is due to the heterogeneity of the structure and composition of the hard dental tissues, the hydrophilicity of the exposed dentin surface, smear layer formed during the preparation of the cavity and the characteristics of the adhesive system, such as its physical and chemical properties and the way it interacts with enamel and dentin. With the development of technologies, adhesive systems have evolved from systems that do not involve the etching of hard dental tissues to systems with preliminary total etching and self-etching adhesive systems.

Summary:

Objective: to develop a questionnaire for assessing the level of dentophobia and dynamics of the relationship in the «doctor-patient» in the course of treatment and to conduct its clinical trials. Material and Methods. 90 patients aged 26-56 years with chronic generalized periodontitis have been examined. The research has been conducted by Hospital Anxiety and depression (HADS) questionnaire to determine the level of dentophobia; psychophysiological testing was carried out on the hardware-software complex (APC) «NS-Psychotest» (LLC «Neurosoft», Russia, Ivanovo, 2007). Clinical examination included examination of the mouth and the index of assessment of hard tissue of teeth and peri-odontal tissue: a defining simplified oral hygiene index (UIG), sulcus bleeding index (IR), gingival index (PMA). The survey was carried out before the treatment and 3 months after the treatment. Statistical analysis was performed in the program Statistica. Using non-parametric methods (Wilcoxon test, criterion A2, Spearman correlation coefficient). Statistically significant differences with p < 0,05 were considered. Results. The development of the questionnaire was carried out in accordance with all generally accepted standards. The initial testing was performed easily by patients. During the clinical trials visible results of the proposed questionnaire were obtained that proved its sensitivity to the changes in dental and psychological status of the oral cavity. Conclusion. The practical application of the questionnaire determines the level of dentophobia during the first visit. The data of the questionnaire identify the differentiated deontological approach. It improves the relationship with the patient, reduces his level of dentophobia. The reduced dentophobia promotes compliance and psychologically comfortable communication in the doctor-patient system, which also contributes to the prevention of emotional burn out among dentists.

Authors: Ivaschenko Y.Y., Shvarts Y.G., Parkhonyuk E.V., Eremin O.V.
Organization: Saratov State Medical University
Summary:

Objective: to establish the relationship of chronic generalized periodontitis, multiple cavities and dentofacial anomalies with various forms of coronary heart disease and its complications. Material and methods. The study included 294 patients with coronary heart disease and its complications. The patients were divided into two groups. Group I included 89 patients with acute Q-myocardial infarction, which was placed in the presence of at least 2 of the following criteria identified on the basis of a comprehensive clinical and instrumental examination: clinical, laboratory confirmation (CK-MB), electrocardiographic signs of damage or myocardial necrosis. The following statistical methods were used: multi-variate and univariate analysis of variance, non-parametric tests, crosstabulation, chi-square test, Fisher»s exact test. As a measure of variability of the normal distribution standard deviation was used. Results. It is noted that in patients with myocardial infarction more likely than in patients without coronary heart attack in history severe generalized periodontitis, dentofacial anomalies and multiple dental caries have appeared. In patients with acute myocardial infarction, severe periodontal disease has been associated with increased fibrinogen levels in the blood and an increase in the dispersion of the interval QT, which are known to be indicators of poor prognosis in acute coronary disease.

Comorbidity Of Periodontal And Gastrointestinal Diseases

Year: 2009, volume 5 Issue: №3 Pages: 393-398
Heading: Stomatology Article type: Review
Authors: O.V. Eremin, A.V. Lepilin, I.V. Kozlova, D.V. Kargin
Organization: Saratov State Medical University, Saratov Clinical Hospital №3
Summary:

The goal of this article is to present review of literature data concerning the problem of combined periodontal lesions and chronic diseases of digestive apparatus from clinical and laboratory-instrumental view point. Gastrointestinal diseases are frequently accompanied by pathologic changes of periodontium. The leading part of such syntropy is disturbance of a number of regulative mechanisms: immune and endocrine disbalance; microcirculatory and neurohumoral regulation; psychosomatic relations; changes in metabolism of connective tissues; mineral metabolism; vitamin deficit. Pathology of digestive system promotes oral microflora negative influence on periodontium, gingivitis and periodontitis development by means of decreasing nonspecific resistance of organism. Pathology of gastrointestinal tract is the risk factor for development and unfavorable clinical course of chronic inflammatory periodontal diseases
