Clinical features of psoriasis in patients with chronic viral hepatitis and HIV infection

Year: 2018, volume 14 Issue: №4 Pages: 765-769
Heading: Dermatovenerology Article type: Case report
Authors: Tlish M.M., Kuznetsova T.G., Naatyzh Zh.Yu.
Organization: Kuban State Medical University
Summary:

Psoriasis is a chronic dermatosis which manifests itself as benign hyperproliferation of epithelial cells with disrupted differentiation and keratinization. In dermatology polymorbidity is quite widespread and relevant, but at the same time it is not sufficiently studied. Psoriasis in patients with HIV infection is often very severe and poorly amenable to standard methods of therapy, which makes study of clinical and immunological specifics of psoriasis natural history in such patients relevant. The disease significantly worsens quality of life, leads to disability and impairs social life of patients. The paper presents a clinical case in which altered clinical presentation and aggressiveness of psoriasis are related to comorbidities, which requires interdisciplinary therapeutic and diagnostic approach.

Modern aspects of progression of onychomycoses of feet in the Krasnodar region

Year: 2018, volume 14 Issue: №4 Pages: 719-723
Heading: Dermatovenerology Article type: Original article
Authors: Tlish M.M., Shavilova M.E.
Organization: Kuban State Medical University
Summary:

Aim: to study clinical epidemic-logic peculiarities of progression of onychomycosis of feet in the Krasnodar region for the period from 2015 to 2017 Material and Methods: cultural study of subunguial content of 56 patients ill with onycho-mycosis of feet, analysis of annual reports of the State Clinical Dermatovenerologic Dispensary of the Ministry of Public Health of the Krasnodar region for the last 3 years, 298 out-patient medical records, questionnaire of 68 patients with the given disease. Results. An increase in the treatment of patients with onychomycosis in the polyclinic department of the dispensary was noted. The main causative agents are Trichophyton dermatophytes. A nail plate is destroyed more often by the normotrophic type from a distal lateral border. People over 50 are more susceptible to the disease. They request medical assistance mainly in case of progression of profound clinical forms of onychomycosis. Cosmetic defect of the nail is the main noted motif for visiting a doctor. Considerable part of patients, who had undergone the questionnaire, did not worry about possibility to infect other people. In most cases wrong shoes and nonobservance of personal hygiene caused progression of onychomycosis. Dermatomycosis and chronic allergodermathosis were registered more often from the co-morbidity More than 1/3 of patients had been taking antimycotic medications on their own initiative before visiting a doctor. Conclusion. Obtained data allow characterizing a modern patient with onychomycosis as an unapparent circulator of fungal infection. The findings of the study can help a practitioner in development of the most effective preventive measures and choice of optimal therapeutic tactics.

Etiopathogenetic aspects of development of a microbial eczema

Year: 2018, volume 14 Issue: №4 Pages: 651-656
Heading: Skin Diseases Article type: Review
Authors: Tlish M.M., Popandopulo Е.К.
Organization: Kuban State Medical University
Summary:

The relevance of studying of an etiology and pathogenesis of a microbial eczema is caused by the fact that it is one of the most widespread chronic dermatosises which exerts stress impact and leads to essential restrictions in all spheres of activity of the person. In the last decade earlier beginning of a dermatosis, the increasing weight, synchronization of a current with a frequent long recurrence, the considerable distribution of skin pathological process and also formation of resistance to the standard methods of therapy is noted. The data of the Russian and foreign authors concerning the main questions of an etiology and a pathogenesis of a microbial eczema are presented in the review. The epidemiology of this pathology in population is described. General antioxidant activity of blood of patients with limited sclerodermia and its correction

Year: 2017, volume 13 Issue: №3 Pages: 640-643
Heading: Dermatovenerology Article type: Original article
Authors: Tlish M.M., Sorokina N.V.
Organization: Kuban State Medical University
Summary:

Objective: to increase efficiency and to reduce the time of treatment of patients with circumscribed sclerodermia. Material and Methods. 60 patients with circumscribed sclerodermia were under supervision. Two groups of these patients were formed with method of adaptive randomization. In the comparison group the patients received standard therapy according to the federal clinical guidelines. In the general group the standard complex treatment with additional antioxidant therapy was underwent. The course of treatment in all patients was 21 days. The results of treatment were evaluated in accordance with the dynamics of cutaneous pathological process and the concentration of common an-tioxidants. Results. Based on the obtained results of the assessment of the dynamics of the cutaneous pathological process, it is obvious that the regress of dermatosis was more rapid and more active in the general group and was accompanied by a more pronounced increase in the total antioxidant activity of the blood. Conclusion. The proposed method of treatment, having an antioxidant effect and high clinical efficacy, enhances the general antioxidant activity of the blood and allows reducing the duration of treatment of patients with circumscribed scleroderma and, as a consequence, the course of systemic and local corticosteroid therapy.

The patients were subdivided into age groups in accordance with physiological age gradation. The Classification of lichen planus proposed by the Federal clinical guidelines for the management of patients with lichen planus (Moscow, 2015) was used. The diagnosis in each case was confirmed by histological examination of biopsy specimens of the skin. The data on the prevalence of lichen planus was obtained from the annual reports of the Clinical dermatovenerology dispensary of Krasnodar city from year 2012 to 2015. Results. Women were found to suffer

Cytokine markers of the efficiency of correction of oxidative stress in patients with limited sclerodermia

Year: 2015, volume 11 Issue: №3 Pages: 410-414
Heading: Dermatovenerology Article type: Short message
Authors: Tlish M.M., Sorokina N.V.
Organization: Kuban State Medical University
Summary:

Objective: to improve treatment of patients with limited sclerodermia (LSD) by increasing efficiency and reducing the time of their treatment. Material and methods. The examination and treatment of 52 patients with circumscribed scleroderma were conducted. Two groups of these patients were formed with method of adaptive randomization. In the control group complex treatment was underwent according to the standards in this nosology. In the general group the patients additionally received antioxidant therapy. The course of treatment was 21 days in both groups. Results. Treatment results were evaluated in accordance with the dynamics of cutaneous pathological process and cytokine profile. In the course of treatment positive clinical dynamics in patients of the study group went faster and was accompanied by a more pronounced reduction in inflammatory activity in the blood serum. Conclusion. The results suggest that antioxidant therapy improves the efficiency and reduces the treatment time of patients with circumscribed sclerodermia.
