Summary:

Leukemia in children (leukemia) is a systemic hemoblastosis, accompanied by a violation of bone marrow hemato- poiesis and replacement of normal blood cells with immature blast cells of the leukocyte series. Acute megakaryoblastic leukemia-one of the rarest forms of acute leukemia (1-3% of all leukaemias), which is characterized by the presence in the blood and bone marrow, along with undifferentiated blasts also megakaryoblasts, ugly megakaryocytes and platelet clusters. The work presents a clinical case of congenital acute megakaryoblastic leukemia in the child S., combined with the mosaic form of Down syndrome and a congenital heart defect.

Inadequate motivation for maternity is the risk factor for a perinatal fetus pathology and a newborn

Summary:

One of the important social risk factors for the birth of a child with pathology in our time is tobacco smoking, with both mother and father. Goal: assess the formation of motivation for motherhood by questioning women with tobacco. Material and Methods. To carry out the research, we used the "Methods of Investigating Motives for Preserving Pregnancy" according to L. N. Rabovalyuk, N.A. Kravtsovoy; methodology "Motives for the conception of a child" by OA. Urusova. Results. When analyzing the results using the method "Motives for conceiving a child" (OA Urusova), in 45.7% of the women studied, an inadequate type of motivation for motherhood was revealed. An unconstructive motive is the high desire of the subject to have a being that will embody her unrealized dreams and plans. 54.3% of mothers, as the predominant type, have an adequate type of motivation for maternity. In addition, the motives for responsibility for the life of the child, the joy of motherhood, the awareness of the child's own values, the desire to have it for the sake of positive life goals and himself are revealed. Conclusion. Timely detection of inadequate motivation for motherhood and proper prioritization will reduce the percentage of pathological pregnancy, increase the birth rate of healthy newborns.

Health status of newborns after vacuum extraction

Objective: to study the outcome of labor and neonatal status after obstetric operations of vacuum extraction method KIWI. Material and Methods. The research and treatment of 58 newborns after the operation method KIWI were carried out. Conducted survey and evaluation of neurological status of newborn, analysis of medical documents (medical card of the pregnant, findings of ultrasound screening of the fetus, birth history and development of the newborn) were performed. Results. Indications for obstetrics KIWI included: narrow pelvis (17.2%), uterine inertia and discoordination (25.9%), fetal distress — 33 (56.9%). The KIWI application was a complete opening of the uterine mouth, the absence of membranes, a living fetus, clinical according to the size of the fetal head and pelvis of mother. The results of the assessment of children by Apgar scale: 1-3 points from 1.7%; 3-6 points of 25.9%; 7 or more points 72.4% of children. Cerebral ischemia 1 degree was diagnosed in 63.8%; of the 2 degree of 34.5%, grade 3 in 1.7% of children. The average stay of children in the Department of physiology was 6.5 days, the 2nd stage is 11.6 days. Generic tumor from the apparatus KIWI was observed in 36.2%, cephalohematoma in 31,0%; no serious complications have to 32.7% of children. All patients were discharged from hospital. Examination of 29 children at the Department of Catamnesis showed that 13.7% of them noted a neurological deficit in the form of small brain dysfunctions. Conclusion. The introduction of new models of vacuum extraction, enhance the delivery through natural patrimonial ways. No significant impact on the course of the neonatal period, which allows us to recommend the method for more widespread use. However, the vacuum extraction of the fetus may be effective and safe only if indications, conditions and technique are strictly followed.

Summary:

Palivizumab is known as the world's first drug with proven efficacy for passive immunization of severe infections of lower respiratory tract in preterm infants. The paper describes a clinical case of application of palivizumab (sinagis) at a newborn infant with bronchopulmonary dysplasia complicated by episodes of obstructive bronchitis. Particular attention is paid to the mechanism of action of palivizumab on the respiratory syncytial virus providing the data on a clinical picture of respiratory syncytial virus infection in a premature baby and showing the effectiveness of the drug in infants.

Summary:

The article describes a clinical case of a child with an alveolar-capillary dysplasia of the lungs — a rare cause of pulmonary hypertension at a newborn. This pathology is of particular interest from a clinical point of view as it rarely occurs in the clinical practice of a neonatologist. It should be noted that the prenatal diagnosis of this disease is of significant difficulty. The authors found it necessary to highlight the cause of development, the clinical presentation and the methods of diagnostics of alveolar-capillary dysplasia of the lungs.

Premature infants' health at multiple induced pregnancy.

Summary:

Objective: to define the risk factors adversely influencing prenatal development at premature birth at use of methods of assisted reproductive technology (ART); to estimate premature' infants health from multiple induced pregnancy according to Perinatal Center of Saratov for last 3 years. Material and Methods. Under supervision there were 139 pregnant women with application ART. 202 children (51 twins were born and 5 triplet babies), from them 83 premature infants born from multiple induced pregnancy have been analyzed. Results. The newborns examined by method ART, were distributed as follows: 22-28 weeks — 19 children; 29-32 weeks — 23; 33-36 weeks — 41. Asphyxia at birth was marked at all premature infants. Respiratory insufficiency at birth is revealed in 87,3% of cases. The most frequent pathologies in premature infants are revealed: neurologic infringements and bronchopulmonary pathology occured at all children, developmental anomaly — 33, 8%, retinopathies in premature infants — 26,5%. The mortality causes include: extreme immaturity, cerebral leukomalacia, IVN 3 degrees. Conclusion. The risk factors, premature birth at application of methods ART are revealed: aged primiparas, pharmacological influence, absence of physiological conditions of prenatal development; multifetation. The high percent of birth of children with ELBW and ULBW is revealed. RDCN with further BPD development, retinopathies in premature infants and CNS defeat is more often occured.

Summary:

The aim of the article is to present clinical experience of conducting the patient with a genetic syndrome of Langer — Saldino in branch of resuscitation and intensive therapy of newborns of Perinatal Center of Saratov. It is necessary to notice that the described pathology is a rare autosomal dominant disease shown by plural developmental anomalies and determines great attention from the practical point of view. The given syndrome concerns mortality diseases leading to lethal outcomes on early terms of prenatal development, or in the nearest neonatal period from respiratory insufficiency.

Evaluation of knowledge of breastfeeding mothers before and after the maternity school

Summary:

Objective: to evaluate the level of knowledge of surveyed women about breastfeeding before and after the maternity school. Material and methods. The anonymous survey of 300 women on breastfeeding has been conducted. The questionnaire consisted of two parts: theoretical and practical. The questionnaire has been filled in on the 3d day after the childbirth (before the maternity course). On the 4th day after the childbirth mothers have been provided with the main issues on breastfeeding and practical recommendations. Results. The questionnaire survey has showed the low level of knowledge of mothers about breastfeeding before the maternity school. The level of knowledge after the school has increased in 2.5 times. The knowledge in practical aspects has increased in 5.2 times. Conclusion. The courses on breastfeeding contribute to the educational status of mothers concerning the questions of correct lactation dominant.
