Immunological disorders in formation of periodontal diseases at pregnant women

Year: 2010, volume 6 Issue: №2 Pages: 392-396
Heading: Stomatology Article type: Original article
Authors: A.V. Lepilin, M.V. Dubrovskaya
Organization: Saratov State Medical University
Summary:

The research goal is to study clinical and immunological features of parodentium and cytokine profile in oral cavity of pregnant women.
The condition of parodentium tissues was studied at 200 women with physiological pregnancy and 300 women with pregnancy complicated by gestosis. According to the results of examination 50 women with gestosis and 50 women with physiological pregnancy had inflammatory periodontal diseases. Phenotyping of lymphocytes by immunofluorescence method, investigation of necrosis containing factor of tumour-a, interleukin-8, interleukin-4 and transforming growth factor beta-1 in oral cavity by immunofermental analysis were performed.
Frequency and character of inflammatory periodontal diseases at pregnancy were defined. Correlation of gingivitis and periodontitis at pregnancy with extragenital pathology was demonstrated. Immune and cytokine disbalance contributed greatly to pathogenesis of inflammatory periodontal diseases at pregnant women.
Thus pathogenesis of oral hygiene, smoking, gestosis, immunosuppression and cytokine disbalance affects inflammatory periodontal diseases at pregnant women

Comorbidity Of Periodontal And Gastrointestinal Diseases

Year: 2009, volume 5 Issue: №3 Pages: 393-398
Heading: Stomatology Article type: Review
Authors: O.V. Eremin, A.V. Lepilin, I.V. Kozlova, D.V. Kargin
Organization: Saratov State Medical University, Saratov Clinical Hospital №3
Summary:

The goal of this article is to present review of literature data concerning the problem of combined periodontal lesions and chronic diseases of digestive apparatus from clinical and laboratory-instrumental view point. Gastrointestinal diseases are frequently accompanied by pathologic changes of periodontium. The leading part of such syntropy is disturbance of a number of regulative mechanisms: immune and endocrine disbalance; microcirculatory and neurohumoral regulation; psychosomatic relations; changes in metabolism of connective tissues; mineral metabolism; vitamin deficit. Pathology of digestive system promotes oral microflora negative influence on periodontium, gingivitis and periodontitis development by means of decreasing nonspecific resistance of organism. Pathology of gastrointestinal tract is the risk factor for development and unfavorable clinical course of chronic inflammatory periodontal diseases

CLINICO-DIAGNOSTIC IMPORTANCE OF NITRIC OXIDE SYNTHASE POSITIVE CELLS OF GINGIVAL TISSUES AND STOMACH IN PATIENTS WITH PERIODONTAL DISEASES AND PEPTIC DUODENAL ULCER

Year: 2007, volume 3 Issue: №4 Pages: 73-76
Heading: Stomatology Article type: Original article
Authors: Lepilin A.V., Ostrovskaya L.Yu., Karabushina Ya.G., Islamova E.A.
Organization: Saratov State Medical University
Summary:

Clinical features of inflammatory periodontal diseases in patients with peptic duodenal ulcer were studied. The severity if inflammatory periodontal diseases increases according to hyperplasia of nitric oxide synthase producing epithelial cells of gingival. It was shown, that the increasing of quantity of nitric oxide synthase-like cells of gingival and stomach is a risk factor of associated development of inflammatory periodontal diseases and peptic duodenal ulcer. Results of morphometrical analysis of nitric oxide synthase-like cells of gingival can be used to verify the degree of severity and effectiveness of therapy of periodontal diseases.
Key words: periodontits, gingivitis, peptic duodenal ulcer, nitric oxide synthase.

THE USE OF ULTRASONIC SYSTEM “VECTOR” IN TREATMENT OF CHRONIC GENERALIZED PERIODONTITIS

Year: 2008, volume 4 Issue: №2 Pages: 132-136
Heading: Stomatology Article type: Original article
Authors: S.A. Vostrikova, A.V. Lepilin, Ya.G. Karabushina,, E.K. Makletsova
Organization: Saratov State Medical University, Saratov Stomatologic Clinic «Denta-Med»
Summary:

Results oftreatment ofpatients with periodontitis using ultrasonic system "Vector"or sealer and Grace! curetes were compared. Microflora of periodontal pockets and gingival mastcells were studied. It is shown that the use of ultrasonic system “Vector" has clinical morphological efficiency and allows to achieve stable remission of periodontitis during 6 months of watching at 91,2% patients. After periodontitis treatment with use of sealer and Gracei curetes functional activity of mast cells was increased and relapse of periodontitis at 30% patients developed.

THE CONDITION OF PERIODONTAL TISSUES IN PATIENTS WITH MANDIBULAR FRACTURES IN COMBINATION WITH INFLAMMATORY DISEASES OF PERIODONTIUM IN DYNAMICS OF TREATMENT

Year: 2008, volume 4 Issue: №1 Pages: 115-118
Heading: Stomatology Article type: Original article
Authors: A.V. Lepilin, N.L. Erokina, V.A. Titorenko, L.U. Ostrovskaja, H.U. Bisultanov
Organization: Saratov State Medical University
Summary:

The immobilization of broken fragments by two-jaw anchor splints in patients with the mandibular fractures in a combination with inflammatory diseases ofperiodontium usually causes the exacerbation and progression of the diseases and growing progressively worsening ofperiodontium status. The intensity of these conditions depends on an initial status ofperiodontal tissue. The posttraumatic suppurative inflammatory complications of the mandibular fractures frequency depending on the initial stage of periodontal disease are marked.
