Summary:

The research goal is to confront the duration of withholding operative therapy of obstructed inguinal hernias with peculiarities of medical and social and psychological status of patients. Materials and methods: 186 patients with inguinal hernias aged from 25 to 60 years were examined. Duration of withholding operative therapy was determined from the moment of officially established diagnosis to the moment of admission to surgical hospital. Results: Medical aid to patients with inguinal hernias who went to the hospital by themselves is no more than 29% and in most cases (53,6%) diagnosis is officially established while staying in health care institutions with other diseases, 94,6% of patients realized the fact that full recovery from inguinal hernias was possible only after surgical procedures. Conclusion: In the first year after establishment of diagnosis consent to surgical treatment of inguinal hernias is associated with the age under 30, higher education, the presence of partially or entirely unreduced inguinal hernia, prevalence of such qualities of charac-
ter as psychasthenia, paranoia, hypomania. In five-six years after official establishment of diagnosis patients aged over 50 years with higher education and prevailing of such qualities of character as hypohondria, depression, psychasthenia give their consent to surgical treatment of inguinal hernia regardless of its type and size.

Dynamic regulation of cerebral blood flow and it's clinical, and laboratory markers in elderly patients with arterial hypertension

Year: 2011, volume 7 Issue: №4 Pages: 842-845
Heading: Internal Diseases Article type: Original article
Authors: Malinova L.I., Sadjaya L.A., Tikhonova L.A.
Organization: Saratov Road Clinical Hospital, Saratov Institute of Cardiology, Russia
Summary:

Aim: to estimate the dynamic regulation of cerebral blood flow (CBF) and significance of clinical, and laboratory parameters as a possible markers of CBF violations. Material. 179 elderly patients with arterial hypertension were involved in the study. Transcranial Doppler ultrasonography (Philips Envisor HD, USA) was used for CBF parameters dynamics during orthostatic hypotension. Results. Main directions of CBF parameters changings (Friedman ANOVA p 0,0091 ... 0,012, coeff. of concordance 0,308 ... 0,691) were detected. Gender differences of CBF parameters, as well as blood pressure level influence upon CBF regulation during orthostatic hypertension were studied. Conclusion. Received data allow us to consider gender, controlled or uncontrolled hypertension, cytomorphological parameters of peripheral blood, diuretics usage as clinical, and laboratory markers of dynamic regulation of CBF in elderly patients with arterial hypertension

Research results of application of reflexo-therapy in correction of myopia

Year: 2011, volume 7 Issue: №3 Pages: 624-628
Heading: Ophtalmology Article type: Original article
Authors: Agasarov L.G., Yurova O.V., Frantsuzov A.S., Tikhonova L.A.
Organization: Saratov Road Clinical Hospital, Russian Scientific Center of Restorative Medicine and Health Resort, First Moscow State Medical University n.a. I.M. Sechenov
Summary:

Objective: To develop the optimum scheme of acupuncture, aimed at improving the functional status of the visual analyzer in patients with myopia. Materials. Treatment was conducted in the main group by reflexology courses in 10 procedures and basic therapy in the control group, only basic therapy. The examination included the classic ophthal-mologic research methods, ultrasound and Doppler echoofthalmografy with color mapping of blood vessels — the ophthalmic artery (OA), central retinal artery (CRA) and posterior long ciliary arteries (ZDTSA), the study of visual efficiency and visual assessment of the subjective expression of asthenopia. The results of these studies were subjected to statistical analysis using the method of variation statistics with the computation of the arithmetic mean (M), the average error of the arithmetic mean (m) and the level of significance (p). Differences between mean values were considered significant if the probability of errors on the distribution of Student»s less than 0,05. The result showed a pronounced effect method of acupuncture on the clinical features, hemodynamic and ergonomic eye, the severity of visual fatigue and general condition of patients. Conclusion. The use of corporal-auricular acupuncture technique in patients with myopia allowed to increase the visual acuity without correction in 1,6 times, visual acuity with correction in 1,2 times, lower rates of subjective and objective refraction in an average of 29.8 and 32.4%, increase rates of accommodation of more than 1,5 times, to increase blood flow velocity characteristics in the GA by an average of 9.7, the CAC at 33.4 and ZDTSA by 27,5%. All this is possible to reduce the severity of subjective indicators of asthenopia in 58% of cases and to increase visual efficiency by 11,3%

Age dependent dynamics of intima-media complex thickness in elderly patients with arterial hypertension

Year: 2011, volume 7 Issue: №3 Pages: 611-614
Heading: Internal Diseases Article type: Original article
Authors: Sadjaya L.A., Tikhonova LA., Malinova L.I.
Organization: Saratov Road Clinical Hospital, Saratov Institute of Cardiology, Russia
Summary:

Aim: To estimate the dynamics of intima-media complex in elderly patients with arterial hypertension. Materials: 179 elderly patients with arterial hypertension were involved in the study. Mean intima-media wall thickness (IMT) of common carotid arteries in plaque-free sites and prevalence of plaques were evaluated by B-mode ultrasound investigation (Philips Envisor HD, USA). Results: IMT changing was of nonlinear character, remained stable up to 74 years. Mean rate of the following IMT augmentation was 0.157 mm per year. Frequency of atherosclerotic plaque revealing was significantly increased since the 7th decade. Significant correlation between IMT and systolic, diastolic, mean blood pressure levels or medication spectrum was not revealed. Conclusion: Received data proved significant influence of aging upon IMT enlargement
