Analysis of resources of therapeutic institutions providing medical care to patients with diseases of the circulatory system

Summary:

Objective: to conduct an analysis of the provision of beds and medical personnel of medical institutions in the region that render medical care to patients with circulatory disorders. Material and Methods. Calculation of the optimal volume of inpatient medical care, the required number of beds and staff units of cardiologists in the region was carried out in accordance with regulatory legal acts. Results. In medical organizations, an excessive number of round-the-clock beds on the "cardiology" profile (more for 186 beds) and insufficient number of cardiac beds in day hospitals (less than 40 beds) were deployed; the number of cardiologists working in cardiological rooms of polyclinic medical organizations, dispensaries for cardiology and daily hospitals has been identified. Conclusions: Redistribution in the region's medical organizations of the bed fund, material, and financial resources leads to the optimization of patients' referrals and promotes rational use of resources of health care, increasing the availability and improvement of quality of medical care to adult population of the region.

Quality of life and health risk factors in officers departed from Armed Forces

Summary:

Objective: assessment of quality of life, identify of health risk factors for officers discharged from the Armed Forces, and development of step-by-step system for medical and social assistance to reserve soldiers (retired). Material and Methods. Anonymous questioning of 358 officers in different time periods, including the last year of service before departure, with the subsequent processing of the data using the statistical program SPSS. Results. It was established that the highest indicators of quality of life were from the military in reserve (RET.) more than 3 years, and the lowest from the military a year before the departure from the Armed Forces and in the first year after the departure. Conclusion. The authors proposed a step system of health and social care for members of the military reserve (retired) and an algorithm for its implementation, including regulations of interdepartmental interaction and integration of all organizations providing medical care.

Characteristics of cases with poor outcomes of rendering medical care to children in six regions of the Russian Federation

Objective: to determine the characteristics of the outcome of the improper care for children, established during the commission of forensic examinations. Material and Methods. The material of this study was commission a forensic medical examination of the archives department of complex expertise of the Bureaus of Forensic Medicine of Moscow; the Moscow, Penza, Samara and Ulyanovsk regions, and the Republic of Mordovia, conducted from 1996 to 2006. The method of this study was a statistical database processing using Excel software application package. Results. The article presents the results of the analysis of 279 forensic medical examinations conducted by committees in the Bureaus of Forensic Medical Examination of Moscow; the Moscow, Penza, Samara and Ulyanovsk regions, and the Republic of Mordovia from 1996 to 2006. The examinations were conducted in connection with poor outcomes of rendering medical care to children. Conclusion. The number of conducted examinations correlates with the population of the region. The parents of the children affected by poor treatment mostly demand the medical staff to be prosecuted and more often make legal claims to the quality of emergency medical care; dissatisfaction with the quality of medical care is more often expressed by the parents of children under 3 years old. Legal claims are more often made against obstetricians-gynecologists, pediatricians, surgeons, infectious disease specialists and anesthesiologists-resuscitators. If the conclusion of the forensic medical examination committee on the nature of the pathological process coincides with the final clinical diagnosis, the provided medical care often turns out to be adequate; in cases of inadequate medical care the risks of moderate and grievous bodily harm as well as the patient's death are high.

Organization of medical care in adult population with non-production eye injuries.

Summary:

Aim: justify the need for institutional arrangements for the provision of medical care to adults with non-productive eye injuries. Material and Methods: analytical, medical, statistical, and comparative analysis. Results. The problem of eye injuries is unresolved today and has not only medical, but also a great social importance. The study calls the structure revealed that the majority of the adult population is unevenly within 24 hours of the first day sought treatment. So, the peak of appeals noted in the first half of a day. Every fourth victim turned to a medical institution for the first two hours after the injury. Young people under the age of 18 years sought medical help almost half of the cases within two hours after injury (40.6%). In 7.4% of cases, the treatment of ocular injury was performed in a hospital. 99.3% of patients with eye injuries recovered. Criteria of medical care evaluation in daily in-patient department in pediatrics

Summary:

The research goal is to work out criteria for the evaluation of medical care quality. Materials included 386 medical cards of daily in-patients, 216 medical cards of in-patients; 602 cards of analysis of case histories; 4 computer data bases. Methods of mathematical statistics were successfully used in the study. The comparative method of data analysis was applied to the research work. Intensity of medical care in values from 0,1 to 0,5 conditional units corresponded to requirements of criterion of estimation of medical care quality. Parameters of medicinal treatment were close to the standards of treatment in interval from 44,4 to 100%, as criterion of quality of medical care. Specific weight of apparatus and instrumental researches constituted an interval from 7, 4% to 22, 6%, forming corresponding criterion. Interval of effectiveness according to standards of consultations is from 0, 26 to 1, 04 conditional units. In conclusion the article stated that the characteristics for criteria to evaluate medical care in daily in-patient departments were worked out on the basis of indices obtained during the research work
