Adipokines and their possible role in obesity and type 2 diabetes mellitus

Summary:

The adipose tissue is the endocrine organ secreting polypeptide factors, such as adipokines, which are involved in the regulation of feeding behavior, the metabolic, endocrine and inflammatory processes and insulin sensitivity. In obesity and type 2 diabetes mellitus (DM2), the production of adipokines and the activity of their signaling pathways are changed, which plays an important role in the relationship between obesity, insulin resistance and DM2. In the review (according the types of obesity) the data on the production of adipokines with diabetogenic (leptin, visfatin, chemerin, vaspin, apelin, resistin) and antidiabetic (adiponectin, omentin) properties by the adipose tissue, as well as the possible role of adipokines in the development of obesity and DM2 were discussed.

Adherence to recommendations and effectiveness of glucose-lowering treatment in type 2 diabetes patients receiving insulin based in the Nizhny Novgorod region on data of mobile diagnostic and therapeutic centre "Diabetes mellitus"

Summary:

Adherence to treatment is an important factor in diabetes management. Poor adherence may cause ineffective control, especially in insulin-treated patients. Aim of the study: to evaluate the adherence to nutrition, physical activity and self-monitoring in T2DM patients who do and do not receive insulin treatment. Materials and methods. 550 T2DM patients were examined as a part of mobile diabetes center in different areas of the Nizhny Novgorod region. Clinical examination results, HbA1c, creatinine and microalbuminuria were assessed. Adherence to treatment was evaluated by questionnaire Diabetes Self-Care Activities. Results. Insulin-treated patients had longer diabetes duration and worse HbA1 с level and similar levels of adherence related to diet, physical activity, foot care. Only for glucose self-monitoring was adherence higher. Best HbA1c values were observed in patients with adherence in middle region of 3-5 days. Conclusion. Long diabetes duration and worse glycemic control in insulin-treated T2DM patients is not associated with better adherence to treatment. This may be viewed as evidence on insufficient patients education and can be a potential target for improvement of glycemic control.

The condition of endocrine system in young men athletes.

Summary:

Aim: to determine the functioning peculiarities of endocrine system of young men athletes. Material and methods. The were examined 84 athletes and 70 cadets of the Marine Academy. The check-up included the following procedures: the ultrasound examination of the thyroid gland, the assessment of hormonal status (determination of concentrations in serum prolactin, cortisol, thyroid stimulating hormone, free thyroxin, antibodies to thyroid peroxidase). Results. It was established that athletes comparing with people not involved in sports, had significantly higher concentrations of thyroid-stimulating hormone, and lower concentrations of cortisol, free thyroxin. In athletes it was revealed a positive correlation between the levels of cortisol, thyroid-stimulating hormone, free thyroxin and prolactin in serum. Conclusion. High TSH level and relatively low values of cortisol can be considered as indicators of the higher performance of an athlete and be regarded as a reflection of optimal adaptation of the pituitary-thyroid and pituitary-adrenal systems to systematic high physical loads.

Assessment of efficiency of application of various analogues of insulin in treatment of diabetes type 2

Summary:

Purpose: to estimate efficiency of treatment of analogues of insulin Glargin and insulin Detemir in patients with diabetes type 2. Material and methods. 147 patients with diabetes type 2 which have been initially receiving combined therapy by oral antihyperglycemic agent and NPH-insulin have been under the study. Level of glycated hemoglobin has been defined in 3 and 6 months for the general assessment of control of aglycemia. Results. According to the study the use of insulin analogues (Detemir, Glargin) versus NPH insulin for 6 months allowed to reach individual target values of glycemia in 70% of patients with diabetes type 2 with a low risk of hypoglycemia. Conclusion. The use in clinical practice of modern insulin analogues and simple algorithm for dose titration, allows simulating as closely as possible the physiological insulin secretion, helping to achieve compensation of carbohydrate metabolism in larger numbers of patients in comparison with NPH insulin.

About the influence of non-drug methods of treatment on reducing body weight in patients with metabolic syndrome

Summary:

The research aims to study influence of various medical complexes on a body weight reduction, the main clinical manifestations and metabolic violations at patients with metabolic syndrome. Material and methods. There had been organized a clinical-functional examination and treatment of 80 patients with various manifestations of metabolic syndrome, aged from 25 to 55 years (39,1 years on the average) with prescription of a metabolic syndrome from 2 to 5 years. Result. Medical complexes with application of physical factors are developed and approved, the efficiency was estimated on indicators of a lipid range (triglycerides, the general cholesterol, lipoproteids of low and high density, (3-proteids and aterogenecity coefficient), and also to level of a glycemia, alanin and asparagin transaminases, creati-nine and urea — to define an extent of violation of metabolic processes. Conclusion. The most expressed results on a reduction of body weight, correction of the main clinical manifestations, metabolic violations and lipid imbalance were received at application of a diet, electrostatic massage from the device "Hivamat", multifactorial polymodal hardware influence from the AlfaLedOxyLight-Spa installation, complex influence on the musculoskeletal device from the Hyuber installation and psychological correction from Shufrid system.

Complex correction of erectile and copulative violations at patients with obesity and reproductive function violation

Summary:

The study aimed the development and assessment of features of corrective action of a medical complex on a lipid imbalance at patients with obesity. Material and methods. For an assessment of features of corrective action of a medical complex on a lipid imbalance at patients with obesity in research I was 50 male patients with obesity and frustration of the reproductive sphere aged from 24 to 68 years are included, middle age was 38,5±6,1 years and 7 healthy persons, men of comparable age without any pathological states, results of which all researches were accepted to values of norm. To all patients included in research, except all-clinical inspection calculation of an index of body weight and the relation of a circle of a waist to a circle of hips, measurement of arterial pressure were applied questioning concerning food and food behavior, anthropometry (growth the body weight, a circle of a waist and hips). Besides all patients conducted laboratory methods the researches including definition of atherogenic fractions of lipids (the general cholesterol, triglycerides, LPNP and LPVP). Researches were conducted before treatment and after a course of treatment. Results. The effective complex program for restoration of reproductive function at patients with obesity is developed. Conclusion. Application of the developed complex program more than its separate components caused the expressed reduction of body weight, mainly due to reduction of fatty tissue and manifestations of visceral obesity in patients with obesity and violation of reproductive function, including due to elimination of a metabolic imbalance.

Siofor influence on the process of lipid peroxidation and antioxidant status at patients with metabolic syndrome

Summary:

The Purpose of the work is to research siofor influence (metformin) on the activity of the process of lipid peroxidation and antioxidant activity of blood serum at patients with metabolic syndrome. Material and Methods. 62 patients with metabolic syndrome at the age from 30 till 60 were examined and treated by siofor (1700 mg per day) during a year. The process of lipid peroxidation was studied due to the level of lipid hydroperoxide of blood serum. Antioxidant capacity was based on the antioxidant reaction in the blood serum with definite number of exogenic hydrogen dioxide (mkmole/l) with the method of ELISA. Results. Intensification of process of lipid peroxidation has been observed at patients with metabolic syndrome —the level of lipid hydroperoxide of blood serum has been 2,9 (1,94; 3,9) mkM, antioxidant activity of blood serum has been decreased — 276,4 (239,0; 379,9) mkmole/l. In 12 months of siofor intake hydroperoxide level has been decreased till 1,1 (0,8; 1,9) mkM, but antioxidant activity has been increased and amounted 320,0 (278,9; 334,3) mkmole/l. Conclusion. Siofor has been proved to be a highly effective medicine for correction of process of lipid peroxidation and for improvement of antioxidant activity of blood serum at patients with metabolic syndrome.

Summary:

We have presented the review of literature devoted to the problem of pancreatogenic diabetes mellitus. We have found up that prevalence of pancreatogenic diabetes mellitus needs to be adjusted. In analyzed literature the influence of localization of structural changes of pancreatic tissue on risk of diabetes mellitus development is studied insufficiently. In the result of our literature analysis we have detected that treatment of pancreatogenic diabetes mellitus up to date is not unified and is mostly descriptive, what indicates on necessity of further investigations in order to optimize and increase the efficacy of its treatment.

Summary:

Aim: the study was devoted to the analysis of efficacy of different methods of treatment of pancreatogenic diabetes mellitus (DM) in alcoholic and biliary forms of chronic pancreatitis (CP). Material and methods. We have observed 86 patients with CP and first diagnosed DM. The assessment of carbohydrate metabolism compensation via HbA1 с level was done using analyzer IMX «Abbot» (USA). Results. At the 1st step of DM treatment we recommended the lifestyle modification and usage of creon 30-60.000 units daily. At the 2d step of treatment if necessary we added glyclazide. Conclusion. We have detected that irrespective of duration of alcoholic form of CP in case of subsequent DM development the lifestyle modification together with replacement ferment therapy in 90% of cases were non-effective. In the result of the research it was established, in case of 3 and more relapses per year in alcoholic form of CPthe probability of achievement of target HbA1 с level in complex with lifestyle changes and replacement ferment therapy was twice low, in connection with that fact the indication of oral anti-diabetic agents became necessary.

Functional state of kidneys in patients with clinical manifestations of hypothyroidism

Summary:

The research goal is to study the function of kidneys and serum concentration of immunoregulatory cytokines (IL-1ГА, IL-6, IL-8, IL-10) in conditions of thyroid hormone deficit. Methods: The character and severity of renal dysfunction were investigated in patients with primary hypothyroidism. The clinical examination included study of serum concentration of creatinine, urea level, potassium and natrium in blood serum, blood and urine analyses, total cholesterol, high and low density lipoproteins, daily microalbuminuria, the performing of Zimnitsky test, urinary excretion of chemokines MCP-1, RANTES, rate calculation of glomerular filtration, thyroid hormone state. Results: The renal function in clinical hypothyroidism without concomitant kidney disturbances was characterized by normal parameters of the concentra-tional kidney function, reduction of glomerular filtration rate, increased of serum creatinine, urea excretion level of chemokine MCP-1 and level of cytokines IL-6, IL-8 in patients with autoimmune hypothyroidism were revealed. Conclusion: It was revealed that autoimmune hypothyroidism effected balance disturbance of cytokine-producing activity of Th1 and Th2 type therefore developing autoimmune state and disease progression
