Perinatal lesions of the central nervous system in newborns according to the data of Clinical perinatal center of the Saratov region

Summary:

The purpose was to analyze the risk factors for the development of perinatal lesions of the central nervous system and to identify cause-effect relationships. Material and Methods. The retrospective analysis of primary documentation: history of childbirth and history of development of newborns is carried out. The work is based on the results of a comprehensive clinical and neurological examination, neurosonographic data and individual laboratory parameters. Results. In 2017 in the clinical perinatal center of the Saratov region 3301 births were conducted, of which the first delivery was performed in 69,4%, repeated in 30,6% and cesarean section in 41,2% of women. Hypoxic-ischemic disorders were observed in 928 newborns (27,7%). Children with low Apgar score had a combined genesis of chronic intrauterine hypoxia and acute asphyxia in childbirth in 3.8% of cases. Intraventricular hemorrhages (IHD) were diagnosed in 4,3% of cases, birth injuries in 3,6%. Congenital malformations of the central nervous system (CNS) were detected in 31 children (24,4%). Summary:

 An accurate assessment of renal function is critical for adequate dosing of medications. The most informative marker reflecting the functional state of the kidneys is glomerular filtration rate (GFR). Determination of inulin clearance is the most accurate way to assess GFR in children and adults. In this case, the method is invasive, cumbersome, younger children require the installation of a urinary catheter. Techniques for radioisotope estimation of GFR can solve this problem, but all widely used radioisotopes have the ability to bind slightly to plasma proteins, which leads to an underestimation of real values.

Urinary Cytokines Spectrum of Different Etiologic and Pathogenic Types at Children with Chronic Pyelonephritis

Summary:

Objective. We investigated the diagnostic role of urinary IL-1b, IL-6 and IL-8 in children with different etiologic and pathogenic variants of chronic pyelonephritis. Material and methods. 132 patients (92 girls and 40 boys) aged 1-17 with chronic pyelonephritis were examined: 18 patients with primary pyelonephritis (I group); 114 with secondary pyelonephritis (II group); 30 patients without kidney disease completed control group. Results. Our data showed increasing level of all mediators of inflammation, especially in acute stage of pyelonephritis and with 10-years anamnesis. We suggested the role of IL-6 in nephrosclerosis. Conclusion. The content of urinary IL-1b, IL-6 reflects the level of kidney inflammation, and IL-8 — the degree of inflammation. We conclude that the level of urinary IL-8 could be a new indicator for nephrosclerosis
