Prenosological risk criteria for the development of bru-xism in young adults

Summary:

The purpose of this study is to assess the dental status of university students to identify the initial risk factors for the development of parafunctional activity of masticatory muscles and bruxism. Material and Methods. Dental examination was conducted in 445 students aged 18 to 25 years. According to the accepted inclusion and exclusion criteria 240 examined person were divided into 2 groups: the first group comprised subjects without revealed functional disorders of the masticatory system (53 persons); the second one —with revealed functional disorders of the masticatory system and damaged dental hard tissues (187 persons). Results. The frequency of detected bruxism cases composed 56%. Hypertony of masticatory muscles is registered in 80.4% cases. Conclusion. Based on the assessment of the students dental status, 43.3% of the examined have prenosological signs of parafunctional activity of the masticatory muscles and functional disorders in the masticatory system, which makes them a risk group for developing bruxism.

Summary:

Ceramic cleavage on fixed orthopedic structures is a very common complication of occlusive trauma. Attempts intraoral repair of such defects have been undertaken since the 90s of the last century. The existing technique, including the treatment of the prosthesis with hydrofluoric acid and the subsequent application of silanes, quite successfully coped with the task at the boundary of the chip within the ceramics. When chipping to metal, this technique does not provide adhesion of the composite to the metal. The situation has changed dramatically with the appearance unsilane primers containing MDP monomer and providing reliable chemical adhesion of the metal to the composite. As follows from the presented clinical case, even after 1 year after the repair of the ceramic prosthesis, the anatomical, functional and aesthetic characteristics of the dentition are fully preserved.

Evaluation of microcirculation parameters in the vessels of tooth pulp in the treatment of dentin and factors affecting its condition

Summary:

Objective: to assess the parameters of microcirculation in the pulp vessels in the immediate and remote periods of observation in the treatment of dentin caries, depending on the depth of destruction of hard tooth tissues, the type of restoration material and methods of filling. Material and Methods. Registration of microcirculation parameters in the vessels of the tooth pulp was carried out by laser specie-optical devices «Speclometer». The study of the state of hemodynamics in the pulp of the tooth was conducted in 233 patients, a total of 286 teeth were sealed. For dental treatment used designed glass ionomer cement light-cured composite materials import manufacture and restoration complex. Results. The reasons leading to a change in blood flow in the pulp of the tooth are due to a violation of the marginal fit of the material to the hard tissues of the tooth, interocclusal and interdental relationships, the influence of both the adhesive system and the restoration material. The restoration of blood flow in the vessels of the tooth pulp in the treatment of dentin caries (cavity of average depth) reaches control values from 14 days to 1 month, with a deep carious cavity from 1 month to 3 months. Restoration of blood flow in the vessels of the tooth pulp in the treatment of deep caries with the modified «sandwich method» reaches control values after 14 days, with the «sandwich method» from 1 to 3 months (p <0.05). Conclusion. The parameters of hemodynamics (blood flow level and intensity) are important predictive criteria assess the status of microcirculation.

Investigation of abnormalities in buccal epithelial cells in patients with lichen planus of the oral mucosa and lips

Summary:

Objective: to study the incidence of buccal epithelial cells with nuclear abnormalities in women aged 50- 60 years, suffering from lichen planus; identification of the spectrum of nuclear aberrations and analysis of diagnostic information content of the studied indicators in assessing the risk of complications. Material and Methods. The incidence of nuclear aberrations in buccal epithelial cells was studied in 20 women aged 50 to 60, patients with lichen planus. 10 women of the same age group without the disease were selected for the control group. The use of ROC-analysis allowed to determine the most informative cytogenetic criteria, allowing to establish the diagnosis of «lichen planus», calculating their critical values, sensitivity and specificity for their use as diagnostic tests. Results. A change in the number of cells with micronuclei and signs of destruction of the nucleus, and cells with nuclear aberrations in patients in the area of disease was revealed. Indicators of nuclear destruction in cells outside the affected area in the group of patients differed from those in the control sample. Conclusion. The increase in the number of cells with micronuclei in the area of lesion with lichen planus is a sign of genetic instability, which can start the process of their malignancy. Recognition of nuclear aberrations against the background of nuclear destruction makes it possible to diagnose lichen planus. Determination of the frequencies revealed perinuclear vacuoles and karyopyknosis in the affected cells of the oral mucosa and lips may serve as a confirmation of diagnosis.

Objective: to assess the impact of behavioral psychotherapy and psychopharmacocorrection with an anxiolytic drug on the level of anxiety and dentophobia, compliance, and the efectiveness of therapy in patients with infamma-tory periodontal diseases. Material and Methods. Comprehensive examination of 40 patients aged 27–57 years with a diagnosis of refractory periodontitis. The clinical assessment of the periodontal tissues was taken into account and the psycho-emotional status of the patient was assessed using the characterological questionnaire H. Smishek, the hospital anxiety and depression scale and a specially designed questionnaire for determining the level of dentophobia and the dynamics of the doctor-patient relationship. Results. Since the frst visit and till the 26th week consecutive decrease in level of uneasiness, then slight increase by the 52nd week is noted. Conclusion. The use of the developed algorithm for personalized treatment of patients with infammatory periodontal diseases with the inclusion of behavioral psychotherapy and psychopharmacocorrection may reduce the level of anxiety and dental fear, increase compliance and the efectiveness of therapy.

Objective: at patients with chronic generalized periodontitis in combination with chronic ischemic heart disease to study the change of cytokines, biomarkers of osteoproliferation and osteoresorption in comparison with the character- istics of endothelial functions and the determine rational approaches to prosthetics. Material and Methods. 90 patients with periodontitis and coronary heart disease, who use metal, metal-ceramic and removable partial dentures using thermoplastic resin were survey. We studied of cytokines, markers of bone metabolism in the oral fuid, evaluated va-sodilating, vasoconstrictor and adhesive endothelial function. Results. The coronary heart disease in combination with chronic periodontitis is characterized by the endothelial dysfunction, which associated with the activity of periodontitis. Biochemical manifestations of periodontitis, bone resorption and endothelial dysfunction are more pronounced by the use patients metal dentures compared to metal-ceramic and removable partial dentures using thermoplastic resin. Conclusion. The use of patients at periodontitis and coronary heart disease metal dentures, in contrast to patients using metal-ceramic and removable partial dentures using thermoplastic resin, is accompanied the increase of biochemical markers of infammation and bone resorption in the oral fuid and the endothelial dysfunction.

Summary:

 Literature review is devoted to the analysis of scientific findings about extraosseous effects of vitamin D on the immune system of the periodontal tissues. It was found that the biological functions of vitamin D (VD) in the body are diverse.Along with the detailed study of the effect on metabolic processes in bone tissue, VD makes a significant contribution to a large number of processes that ensure the activity of the immune system. In addition, VD increases the production of "endogenous antibiotics". This allows us to consider VD as a therapeutic agent that prevents the development of inflammatory and destructive changes in the periodontal tissues in population with simultaneous slowing of bone loss and preservation of bone mass of the dentofacial system. Currently detected hidden hypovitaminosis VD in almost majority of the population is considered a leading factor in the development of periodontal disease and its prevention and treatment is one of the most important tasks of therapeutic dentistry.

Summary:

Purpose: to determine the role of chronic inflammation in deciduous teeth data in the formation of dentigerous (follicular) cysts of the jaws in children; to considerthe pathophysiological mechanism of follicular cysts formation. Material and Methods. Case histories of 134 patients of the department of maxillofacial surgery (dentistry) "Nizhny Novgorod regional children's hospital" from 2003 to 2013 with a diagnosis of "follicular jaw cyst" with different localization. Results. The dental status, specifically centers of chronic inflammation, in the area of follicular cyst formation was assessed. The connection with the ecological situation of the residence area of patients with the prevalence of follicular cysts of the jaws in children was found. Conclusion. The key role of chronic inflammation in the form of complicated forms of caries of temporary teeth for the development of follicular cysts of the jaws. The data of the study (the important role of chronic inflammation of the deciduous teeth in the formation of follicular cysts of the jaws in children) adverse environmental conditions of the place of residence will help to develop principles of prevention and examination for early detection of pathological changes.
