Complications after different surgical methods of setting polypropylene suburethral meshes in women with stress urinary incontinence

Summary:

The aim: to determine the complication's rate, based on a seven-year experience, after the stress urinary incontinence correction in women with different methods of setting suburethral polypropylene meshes. Material and Methods. The analysis of seven years of experience of surgical correction with different methods of setting suburethral meshes in 1260 women with stress urinary incontinence; obturator fixation the mesh inside-outside and outside-inside: supra-pubic fixation the mesh; non-trocar fixation the mesh type mini-sling was carried out. Results. The greatest frequency of complications were identified in the first two years of implementation the setting of suburethral meshes, after that the complication's rate decreased progressively and it was in the range 7-8% the last three years. The frequency of early operative complications at a different setting of suburethral meshes had no statistically significant differences, but there were more the statistically significant intraoperative and mesh-associated complications at suprapubic fixation. Conclusion. The study confirms that the installation of suburethral meshes by the method of suprapubic fixation of mesh is associated with a higher incidence of complications and should be performed only in the target group of patients who are not suitable for surgical correction in safer ways.

Summary:

Research objective: to search new laboratory approaches to the diagnostics of immune forms of reproductive failures. Materials and methods. Retrospective research a case — control of 54 married couples with idiopathic reproductive failures (in the anamnesis — 3 and more spontaneously interrupted pregnancy in the 4-8th weeks) and 47 married couples having two and more children has been conducted. Results. It has been revealed that at the immune form of reproductive failures increase of cells of level A- mononuclear cells, expression of HLDR takes place that promotes tolerance cancellation to allogenic germs and to immune interruption of pregnancy. At reproductive failures female au-toserum positively influences activation of T-lymphocyte (CD3 +/HLADR +) that may lead to the death of half- allogenic germ. Conclusion. Level of expression of CD3 and HLADR on CD45 + of mixed allogenic mononuclear cells of spouses may serve as a diagnostic significant criterion for revealing immune reasons of reproductive failures.

Summary:

Research objective: To define clinical predictors of adverse course of nonspecific bacterial vulvovaginitis (NBV) at girls born from mothers with the undifferentiated forms of dysplasia of connective tissue (UFDCT). Materials: At 157 girls clinical research, bacteriological and bacterioscopy research of leucorrhoea from vagina has been conducted, NBV has been diagnosed at 111 girls. At all girls and at their mothers the immunity has been investigated; at all mothers of girls with NBV phenotypic reveals of UFDCT have been found out. Results: At girls with NBV born from mothers with UFDCT severe course of a disease has been marked; and at mothers with UFDCT and at their children with NBV disorders of humoral immunity have been revealed. Presence of more than 5 stigmas of dysembryogenesis with prevalence of thoracodiaphragmatic, articular, cosmetic syndromes and pathology of organs of vision at mothers of girls with NBV is possible to use as predictors of adverse course of the disease

Summary:

The research goal is to determine the risk of infectious process development in placenta of women with undifferenti-ated forms of dysplasia of connective tissue (UFDCT) by agents - herpes viruses (VHG - virus of herpes simplex and cytomegalovirus).
Persistent infection of VHG was revealed in 133 women in the 1 st trimester of pregnancy. All the patients had phe-notypic manifestations of UFDCT; in the 3d trimester of pregnancy immunogram of the second level, histological study of placenta, identification of antigens of VHG family, serological study of placental blood and isolation of DNA of VHG were carried out. All newborns were clinically and laboratory tested on VHG infection.
As pregnant women with UFDCT had high risk of VHG infection development in placenta, they suffered from placental insufficiency. It resulted in unfavourable perinatal outcomes. VHG infection development was caused by disturbances of cellular immunity.

Summary:

Research objective is to define the pathogenesis of varicous veins of small pelvis in women. at Ultrasonic investigation of venous system of small pelvis has been carried out in 290 pregnant women. It revealed 190 patients with varicose veins of small pelvis (VVSP). By means of V.M. Jakovleva's technique phenotypic menifestation of connective tissue dysplasia was determined in all pregnant women. Biochemical manifestations of connective tissue dysplasia were identified by sialic acid level in blood serum, daily excretion of glycosaminoglycans and oxyproline.
High frequency of clinical and biochemical manifestations of undifferentiated forms of connective tissue dysplasia was revealed in pregnant women with VVSP. Patients with VVSP developed tooth and jaw, facial and locomotor damages. Patients with VVSP characterized by visceral undifferentiated forms of connective tissue dysplasia demonstrated by refraction involvement, ventral hernias, flat feet, varicous veins of lower extremities, hypermobile syndrome, mitral valve prolapse of different degree. Biochemical manifestations of undifferentiated forms of connective tissue dysplasia in pregnant women with VVSP were insignificant
