Antihypertensive therapy influence on clinical and psychological characteristics of patients with hypertensive encephalopathy

Aim: to analyze the influence of antihypertensive therapy (AT) on clinical and psychological characteristics of patients with hypertensive encephalopathy (HE). Materials: 107 patients with HE l-lll were given inadequate AT and therefore were under the study. Diurnal blood pressure monitoring (DBPM), neurological status, asthenic disorder level, life quality (LQ) parameters, sleep quality and attitude-to-disease type were estimated. The patients were re-examined in 6-month period after prescribing an adequate AT. The control group included 30 healthy patients. Results: in the process of AT daily average indices of blood pressure (BP) were able to be matched with the control indices, the number of patients with normal degree of night BP decrease raised. Physical and psychological health constituents were increased. Sleep quality improved. The increase of patients with well-balanced attitude-to-disease type was fixed. Resume: the use of the modern antihypertensive drugs affects the clinical and psychological characteristics of HE patients positively

Age aspects of inguinal hernia relapses

The research goal is to study an age role in relapses of inguinal hernias. Relapses of inguinal hernias in 113 patients have been investigated. It is established that the elder the patient the more frequent relapses of inguinal hernias occur. Causes of relapses of inguinal hernias development are age changes in the inguinal canal structure and the tissues that take place in its formation. The article concludes that causal factors of inguinal hernia relapses are age index and structure changes of inguinal canal. It points out that plasty of posterior wall of inguinal canal is considered as a preventive measure in case of inguinal hernia relapse

Pharmacoeconomic analysis of ischemic stroke therapy in patients with arterial hypertension

Directed administration of medicines in complex treatment of patients with biliary pancreatitis and нсv-infection

The research goal was to estimate efficiency of medicine phosphogliv by the directed administration in complex therapy for patients with biliary pancreatitis and НСV- infection. 62 patients with biliary pancreatitis and НСV-infection were examined. These patients were divided into 2 comparable groups: control group I (50 persons) and basic group II (12 persons). In group I traditional basic therapy of biliary pancreatitis was carried out with injection of phosphogliv intravenously. Patients of group II were given erythrocytes intravenously with average therapeutic dose of phosphogliv. Introduction of medicines in autologous erythrocytes was performed by hypotonic lysis.
Effective influence of directed administration of preparation phosphogliv demonstrated decrease in level of bilirubin in blood, activity of AlAt, AsAt in patients of research group. Side-effects were not revealed.
In conclusion the investigation proved that more effective influence of phosphogliv on level of bilirubin in blood and activity of AlAt, AsAt can be reached in combination with traditional basic therapy.

468 patients aged 14 to 88 years underwent surgery concerning perforative ulcer os stomach and duodenum. 10 patients had symptoms of stenosis including 10 patients with penetration. It is demonstrated that incision of ulcer with duodenoplasty or pyloduodenoplasty permit to treat successfully perforative ulcers and to achieve good functional results.
COMPUTER-TOMOGRAPHICAL FEATURES OF CRANIOCEREBRAL TRAUMA WITH DIFFERENT GENESIS

Analysis of the results of computer tomography (CT) of 247 patients with craniocerebral and mine-explosive trauma with different degree of damage in acute and distant period was provided. Authors revealed, that in acute period of craniocerebral trauma CT may detect different kinds of zones of concussion damage including bilateral damage. Atrophic process, mostly diffusive, may develop in distant period. Both ventricular system as well as subarachnoid space may be involved. Mostly severe atrophic cerebral changes are revealed due to third and fourth types of focal concussion damage of the brain. Mine-explosive damage of the brain is not visualized by CT-scanning in acute period. Generalized type of posttraumatic cerebral atrophy is revealed in patients with disease in distant period.

Bilateral Variability of X-Ray Osteometric Parameters Of Human Metacarpal Bones

Hand roentgenograms of men of mature age have been investigated. Parameters of metacarpal bones length and width of left and right hands have been measured. Displacement of maximal and minimal bilateral asymmetry on II and V rays of hand has been established. The obtained data have been considered from the point of view of fluctuating asymmetry, as an indicator of instability of morphogenesis which has been determined by HOX-genes defining structure of hand in mammals.

The analysis of the treatment results of 319 patients with complicated forms of gastroduodenal ulcer disease has been carried out. The group is formed by the 223 patients over 60 years old and 96 young and middle-aged patients. The description ofpreoperative and postoperative treatmentwith application of the controlled sympathetic medication denervation of the elderly and aged people with complicated forms of gastroduodenal ulcer disease in the preoperative period and the exposure of the abdominal nerve plexus to alternative magnetic field of “Magniter" device have been provided. . The treatment algorithm of elderly and aged people with such pathology has been introduced. The results of the research have revealed reliable decrease in postoperative complications development incidence, postoperative complication development relief, earlier improvement of the clinical, laboratory and instrumental indices in elderly and aged patients with complicated forms of gastroduodenal ulcer disease, and also postoperative mortality decrease and cuts of the patients' stay period in hospital.

Since 1998 to 2007 in the clinic of hospital surgery (based on surgical wards) of Ulianovsk State University the following types of surgical treatmentofpatients with cholelithic disease were carried out: more than 2000 laparoscopic cholecystectomies, 154 mini-laparotomies and 173 traditional laparatomies. Low-invasive surgical treatment of acute cholecystopancreatitis of patients over 60 years due to miniacess is advisable for persons with high degree of operationally anesthetic risk.

ROLE OF TUMOR SUPRESSOR P53 IN GENESIS OF ANGIOGENIC PHENOTYPE OF A NEOPLASM IN COURSE OF OVARIAN CARCINOMA

The parameters of a vascular bed (vascular volume in % andvasculardencityon 1 mm2 of tumor) were estimated on the operational-bioptic aspirate of the primary tumor of 83 patients with the ovary cancer (ОС). The immunohistochemical method with the use of mice monoclonal antibodies p53, clone DO-7, lgG2b(M7001 DakoCytomation) has been applied for the estimation of the level of p53 expression. Based on the estimated parameters we can conclude that there is no distinctive group of differences among the different clinical stages of the ОС. However, there is a weak positive correlation between the volume and density of vessels and the disease stages. At the same time, the quantity of the atypical epithelial cells of the primary tumor that express p53 is significantly increasing in each subsequent stage. The results of the research show the possible p53 inactivation in the tumor cells and the stimulatory role ofp53in the neoangiogenesis in the course of the tumor progression.
