Year: 2014, volume 10 Issue: №1 Pages: 141-144
Heading: Pedagogics and Education Article type: Author's opinion
Authors: Goremykin I.V., Morozov D.A., Filippov Y.V., Deriugina L.A., Gorodkov S.Y., Kulikova T.N., Turkovsky V.B., Dolgov B.V.

Summary:

This article analyzes the first experience in the implementation into the pedagogical process of one of the methods of active training — role playing. The need for intensification of medical education process is linked primarily to the rapid increase in the volume required to provide students with information and the lack of training time. The analysis showed that the game significantly increases the level of students' knowledge, but require substantial modernization of the educational process.

The diagnostic value of biomarkers of inflammation, angiogenesis and fibrogenesis to assess the severity of urodynamic obstruction in children with congenital megaureter

Year: 2012, volume 8 Issue: №4 Pages: 996-1001
Heading: Urology Article type: Original article
Authors: Morozov D.A., Krasnova E.I., Deryugina LA., Zakharova N.B., H'icheva Yu.A.
Organization: Saratov State Medical University, Clinical hospital n.a. S. R. Mirotvortsev SSMU, Moscow Research Institute of Pediatrics and Pediatric Surgery
Summary:

The aim of the study is to determine the diagnostic value of urine biomarkers of inflammation (interleukin-1p, tumor necrosis factor-a), angiogenesis (vascular endothelial growth factor) and fibrogenesis (monocyte chemoattractant protein-1, matrix metalloproteinase-9) to assess the severity of urodynamic obstruction of the ureter. Materials and Methods — Complex investigation of 47 children with congenital nonrefluxing megaureter at the age from 1 month till 11 years is carried. Results. The correlation between the urine level of IL-1p and main laboratory criteria of inflammation occurred, so IL-1p as an integrative indicator of pyelonephritis activity was appeared. The relation between levels of VEGF and MCP-1 and indicators of renal hemodynamics allowed to consider increased levels of these factors as markers of irreversible nephrosclerosis. The urine level of MMP-9 was strongly correlated with parameters of peristalyic activity of the ureter. The group of children with lower peristalyic activity of the megaureter was significantly different from group with normal ureteral peristalsis by the high urine level of MMP-9. We do not have reliable data about the diagnostic value of determination of the urine level of TNF-a in children with congenital megaureter. Conclusion: The urine level of biological markers investigations are very important for clinical practice for determination of pyelonephritis activity (IL-1p), severity of ischemic deterioration of renal parenchyma (VEGF, MCP-1). Further studies are needed to clarify the pathogenic role of MMP-9 in the regulation of peristalyic activity of the ureter during embryonic and postnatal development in patients with congenital nonrefluxing megaureter.

Cystic growths of spleen in children

Year: 2011, volume 7 Issue: №3 Pages: 724-726
Heading: Surgery Article type: Author's opinion
Authors: Morozov D.A., Goremykin I.V., Turkovsky V.B., Napolnikov F.К., Pogosyan K.L., Lukyanenko E.A.
Organization: Saratov State Medical University
Summary:

The effectiveness of different surgical techniques in patients with cystic lesions of spleen is considered in the article. The results of treatment of 9 children aged from 9 to 16 years old during the period from 1998 till 2009 have been analyzed. The examination included ultrasound computerized tomography and MRT Differential diagnosis was made between lymphangiomas, hemangiomas and true cystic growths of spleen. The proper diagnosis was made after his-tological examination. The invasive procedures preserving spleen were preferred. But despite the surgical technique the possibility of cystic recurrence was remained. Percutaneus puncture with draining and long term sclerotherapy performed by laparoscopy control was considered to be a safe procedure

Organization: Saratov State Medical University
Summary:

The present article summarizes modern conceptions of tubulointerstitial fibrosis development mechanisms in children with vesicoureteral reflux. The review deals with the questions of extracellular matrix components accumulation and disintegration, tissue hypoxia, remodeling of blood flow, concentration changes in inflammation, sclerosis and angiogenesis biomarkers in the progress of nephrosclerosis

Causative agents of urinary tracts infection in children with obstructive uropathy

Year: 2010, volume 6 Issue: №2 Pages: 442-446
Heading: Urology Article type: Original article

Organization: First Moscow State Medical University n.a. I.M. Sechenov, Russian Research Anti-Plague Institute «Microbe», Saratov State Medical University
Summary:

167 children with different variants of congenital obstructions of urinary tracts were examined; the causative agents of urinary tracts infections were analyzed in all the cases. The changes of the cytokines levels were determined in those patients which had the more often found causative agents in the analyses. It was mentioned that of the total patients with obstructive uropathy (OU) the fifth part in the absence of bacteriuria had a pathogen in biopsy material (BM) of urinary tracts. In fourth part of cases microorganisms that were detected in BM and urine did not matched. The changes of cytokines levels in urine did not have any species-specific dependence on causative agent

Comparative analysis of perioperative period in children with different corrections of vesicoureteral reflux

Year: 2010, volume 6 Issue: №1 Pages: 182-188
Heading: Urology Article type: Original article
Authors: P.V. Glybochko, A.A. Svistunov, O.L. Morozova, D.A. Morozov, B.V. Dolgov, F.K. Napolnikov
Organization: Saratov State Medical University
Summary:

The article presents the analysis of perioperative period of 52 children with different variants of correction of vesicoureteral reflux. It was based on acute-phase proteins amount, cytokines level and adhesion molecules content in blood and urine. the study revealed that if the correction is performed during the latent phase of chronic obstructive pyelonephritis the preference should be given to the endoscopic operations as they do not lead to acute inflammatory process in the urinary tract

Criteria of Clinical Diagnostics and Choice of Treatment Strategy in Children with Disoders of Sexual Development

Year: 2010, volume 6 Issue: №1 Pages: 178-182

Authors: N.V. Bolotova, D.A. Morozov, N.U. Raygorodskaya, D.A. Zharkov, M.M. Grigorjeva, E.N. Tsmokalyuk
Organization: Saratov State Medical University
Summary:

The number of patients with disorders of sexual development (DSD) has increased in 3.54 times during the last three years. The aim is to define the structure of intersexual disorders in children from Saratov, criteria of gender assignment and surgical approach. During the period of three years (from 2006 till 2008) 58 children with DSD aged 0 - 17 were examined. Assessment of external genitals, cytogenetic investigation, hormonal tests were performed. Imaging studies included ultrasonography, cystourethroscopy and laparoscopy with histological investigation. Considering the data DSD in children from Saratov compose sex chromosome DSD - 21,4 %, Gonadal dysgenesis - 26,2%, ovotesticular DSD - 2,3%, 46,ХХ testicular DSD - 2,3 %, complete androgen insensitivity syndrome - 4,7%. The criteria of gender assignment and surgical approach have been described in the article. The research concludes that the infants with intersexual genitels should be examined in special care centers by multispecialty team. It is nessesary to perform the karyotype test and ultrasonic study in newborns to detect the genetic and gonadal gender of the child
