The article deals with new treatment technique in fractures of shin bones. It combines ideal reduction of damaged bone with reposition device without transition of transosseous elements through muscular masses, adequate fixation of bone fragments and change of biomechanical efforts in apparatus according to reparative osteogenesis phases. It is performed due to application of external fixation apparatus

Fixation of periarticular fractures of proximal femur on biomechanical basis

In the work comparison of two methods of pertrochanteric fracture fixation has been carried out: external one, with the help of the transosseous device and immersed one, by the dynamic femoral system. The investigation of the process of dislocation of fragments has been done on biophantomes. The dynamic compression in the external fixation device increases stability of the biomechanical system «bone-fixator» in 3-4 times. The use of single-stage fracture compression with dynamic femoral system allows to create stable fixation of fragments, adequate for early mobilization of patients

In the present research a comparison of two techniques of transosseous fixation of pertrochanteric fracture has been drawn. By way of example linear relocations of fragments have been measured in 8 biodummies (femurs of cows) in two groups through three series of loadings along the long axis of a femur with a step of 490 N from 980 up to 2940 N. The specific load in the range of 16,08±0,65 N/sm2 — 16,32±0,065 N/sm2 causes linear fragment dislocation within 0,13±0,016 mm — 0,08±0,03 mm. Compressive variant of osteosynthesis improves stability of biomechanical system "a bone-fixator" 3-4 times as much

The apparatus and the technique of osteosynthesis ofdiaphyseal fractures of shin bones are presented in the article. The technique combines the possibility of bone fracture fragments position control at compression and reduction in process of cyclic loads. This treatment technique provides the optimization of reparative osteogenesis conditions, earlier restored support-motor functions of an injured limb and prevents contamination of tissues which surround transosseous elements. The constructive solutions of pin-related osteosynthesis presented in the article are adapted in topographical anatomy aspect according to the location of fractures. The technique has been applied to 124 patients with diaphyseal shin bone fractures. Positive outcomes have been recieved in 99,6% of clinical observations.
