Morphological studies in the diagnosis of primary and secondary bone tumors

The aim: to show the possibility of morphological studies in the diagnosis of primary and secondary tumors of bones. Material and Methods. 105 (72%) patients with primary bone tumors aged from 15 to 66 years and 42 (28%) patients with metastatic bone lesions aged from 42 to 70 years were examined and treated for the period from 2008 till 2015. Material for morphological studies was prepared using an open biopsy tissue slices and a scraping resected tumor during surgery. Soft-tissue component is subjected to cytology. The material for histological study included changes in bone and soft tissue. Results. Giant cell tumor was verified in 45% of cases by histological examination. Multiple myeloma was diagnosed in 15% of patients. Osteogenic sarcoma was diagnosed in 14% of cases. Ewing's sarcoma was diagnosed in 3%, 2% of cases were matched by diagnosed chordoma. According to the data received, cancer metastasis of kidney and lung is mostly diagnosed in men from the group of patients with secondary bone defeat. Metastasis of cancer of the breast in women was predominated. Conclusion. The morphological (histological, cytological) study plays an important role in the diagnosis of bone tumors. The coincidence of the cytological and histological diagnoses was 97%.

Biomarkers content in blood serum of patients with osseous neoformations

The aim of the study was the estimation of several biomarkers in differentiation diagnostics of tumour diseases in osseous system. Material and methods. 160 patients with benign (115) and malignant (45) osseous neoformations were investigated. Neopterine, tumour necrosis factor (TNF-а), interleukin-6, adhesion molecules of vessel endothe-lium (sVCAM-1) and vascular endothelial growth factor (VEGF) levels were identified by ELISA solid-phase test and compared with the levels of 20 apparently healthy individuals. Tumor verification was fulfilled by X-ray, CT, MRI and histomorphological essay. Results. Both benign and malignant osseous tumour patients showed increased levels of neopterine, TNF-а and interleukin-6 being more significant with malignant processes. The level of sVCAM was decreased in patients of both groups and VEGF level corresponded to the control, these indexes did not show any differences between the groups. The assessment of diagnostic value of these cytokines indicates that positive results for TNF and neopterine give the ground for the final diagnostic decision while the same results for interleukin-6 give moderate basement for diagnostic decision. Conclusion. Increased TNF-а and neopterine levels allow substantiating the difference between benign and malignant osseous tumours and improving diagnostic quality.

Vascular endothelial growth factor as a risk predictor of thromboembolic complications in patients with orthopaedic profile

Summary:

Objective: to determine the risk of thromboembolic complications in orthopaedic patients. Material and methods. A survey of 151 patients who had undergone total endoprosthesis of large joints of the lower extremities, aged 25-68, under treatment in Saratov Scientific Research Institute of Orthopedics and Traumatology. Results. The performed research allowed to establish that at values of the content of VEGF-A to 183,6 PG/ml a low risk of thromboembolic complications was predicted and for values above 183,6 PG/ml high risk was prognosed. Conclusion. We have proved high efficiency of using the relationship of VEGF and PAI-1 in serum, which plays a leading role in the development of predisposition to thrombosis.

Markers of systemic inflammatory response in coxarthrosis

Objective: to detect markers of the systemic inflammatory response syndrome in patients with coxarthrosis by means of assessment of the status of the hemostasis system, endothelium function and inflammation intensity. Material and Methods. The indices of the plasmatic hemostasis, levels of VCAM-1, ICAM-1, ELAM-1, VEGF-A, neop-terin were analyzed. Results. It has been found that among the patients with hip coxarthrosis a group of patients (47%) with endothelium dysfunction, cellular immunity activation, a high content of Soluble Fibrin Monomer Complex and D-dimers and a group of patients (53%) without any abnormalities in these parameters should be differentiated. Conclusion. Among the patients with hip coxarthrosis a group with signs of a systemic inflammatory response syndrome (occurrence of endothelium dysfunction, cellular immunity activation, a high content of Soluble Fibrin Monomer Complex and D-dimers) and a group of patients with reference values of these parameters can be determined. The markers of the systemic inflammatory response syndrome in cases with hip osteoarthrosis are Soluble Fibrin Monomer Complex and D-dimers, high levels of intercellular adhesion molecule-1 (slCAM-1), vascular cell adhesion molecule-2 (sVCAM-1), cell adhesion E-selectin-1 (ELAM-1) and Neopterin (Np).

The purpose of the article is to evaluate the state of metabolic status of patients with distal onycholysis of hands. Materials and methods: Evaluation of hemoglobin, red blood cells, neutrophil granulocytes, the level of glycolytic processes of lactate and pyruvate, the activity of superoxide dismutase, catalase, myeloperoxidase, neutrophil elastase, serum alkaline phosphatase activity, as well as degradation products content of collagen structures to relative fractions of the blood free and peptide bound oxyproline. Results: It has been shown that free fraction oxyproline in 18 women with distal onycholysis of hands and hemoglobinemia prevails over peptide bound oxyproline fraction in blood, activated glycolytic processes on the modulation mechanism of adaptation with a slight increase of the lactate and pyruvate, the inhibition of activity of antioxidant enzymes, alkaline phosphatase and serum first phase elongation of blood coagulation as a result of the activated plasma recalcification time. Conclusion: The functional state of red blood cells, neutrophil granulocytes in the oppression of the first line of antioxidant enzyme defense and extension in patients with distal onycholysis of hands points to the formation of a mixed type of chronic hypoxia.

Application of self-learning neural network in obstetrics and neonatal practice

Summary:

The purpose of the study is to examine the feasibility of a self-learning neural network to predict the risk of hemo-static disorders in newborns and their mothers. Material: To solve the problems 214 patients have been under the study, of which — 107 women on a day of labor and 107 of their infants. Morphofunctional state of platelets was assessed by automatic hematological analysis, computer cytomorphometry and platelet aggregation. These laboratory studies of patients were used as a training set for a neural network. This study used a neural network with classical structure «layer perceptron.» Results: Testing of the neural network was carried out on two female patients with physiological pregnancy and signs of imbalance of the hemostatic system. At the output of neural network based on laboratory confirmed the presence or absence of pathology of the hemostatic system in two specific patients and their newborns. Conclusion: The introduction of practical obstetric program being developed based on the neural network will reduce and prevent the development of thrombohemorrhagic complications, the protocol will optimize diagnostic and treatment activities, and an opportunity for the state of hemostasis monitor on a background of pathogenetic therapy for women and their newborns during childbirth and neonatal period.

Characteristics of platelet aggregative ability at patients with brain infarction and marked stenosis of cerebral arteries

The central part of brain infarction (Bl) pathogenesis is hemostatic blood activation. Aim: to study platelet aggregative ability in dynamics of acute Bl at the marked stenosis of cerebral arteries (>50%). Materials and methods. Investigation included 39 patients at acute Bl with average age 73,9±8,3 years. Control group consisted of 18 healthy persons with average age 60,2±6,8 years. Evaluation of platelet aggregation was performed with application of light-transmission aggregometer «SOLAR» (RB) and aggregation inductors — solutions of adenosine diphosphate with concentration of 0,5 и 2,0 umol/l and collagen of 2,0mg/ml concentration. Results. Significant hyperaggregation platelet reaction on the 1 st and 10th days of hospitalization without positive changes under treatment has been revealed. Conclusion. Thus, obtained results prove the necessity of monitoring of individual patients sensitivity to antiplatelet therapy in secondary prevention of ischemic stroke with optical aggregometry and inductors complex.

Summary:

The research goal is to study the composition of biological vascular endothelial injury markers in dynamic development of burn disease. Materials and methods: the investigation included 32 patients with thermal burns. The markers of vascular endothelial growth factor (VEGF), monocyte chemotactic protein-1 (MCP-1) and the number of circulating endothelial cells (CECs) were observed. Results: it was revealed that levels of VEGF, MCP-1 and CECs were significantly increased in patients with thermal traumas. Conclusion: VEGF and MCP-1 were proved to be early and prominent vascular endothelial injury markers in burn disease

Summary:

Aim. The study considers the influence of sensitive test-cultures on antagonistic activity and production antimicrobial factors by indigenous and probiotic microorganisms. Material. The original method has been used. It is based on determination of survival of sensitive test-culture under the action of cultural liquids of antagonist processed by metabolites and peptidoglycan of sensitive culture. Resalts. The important role of sensitive test-culture in manifestation of the antagonism active microorganisms has been proved. Received data have proved that antagonistic bacterial activity is a result of cross-species interaction between active and sensitive cultures. The effectors are antimicrobial factors and their regulators. The ability of bacterial peptidoglycan to regulate the cross-species interaction between prokaryotes has been firstly shown. Conclusion. The article concludes that the investigation has revealed new mechanisms of colonization resistance of biotope. It has opened prospects of development of principal new probiotic, prebiotic, sinbiotic, biologically active additive and functional products on the basis of microbial stimulators of bacterial antagonism
