Optimization of chronic kidney disease stages in patients with type 2 diabetes mellitus with the program "ChronicKidneyDiseaseDiagnostics"

Summary:

Objective: to evaluate the practical possibility of automated calculation of glomerular filtration rate (GFR) in patients with type 2 diabetes mellitus (DM) on the basis of the developed computer program «Chronic_Kidney_Disease_Di-agnostics». Material and Methods. The study included 233 patients (78 men and 155 women) with type 2 diabetes who were hospitalized in the Neuroendocrinological Department of Nizhny Novgorod Regional Clinical Hospital n.a. O. N. Semashko during the period from 2013 to 2016. The average age of patients was 57.75 years [52; 64]. According to the history the mean duration of type 2 diabetes was of 5.3 years [0.3; 8.0]. The average level of glycosylated hemoglobin (HbA1c) was 9.53% [7.0; 10.2]. All patients underwent clinical and laboratory an3 instrumental studies. The data obtained were entered into the program «Chronic_Kidney_Disease_Diagnostics» (authors: N.A. Yarkova, N.N. Borovkov, D.A. Terekhova). Statistical analysis of the results of research carried out by Statistica 7.0. Results. According to the study in patients with type 2 diabetes, chronic kidney disease has been identified in the pre-clinical stages C2 and C1 (56.2 and 25.8%, respectively). Conclusion. The program «Chronic_Kidney_Disease_Diagnostics» is intended for nephrologists, endocrinologists and therapists. It allows specialists to diagnose kidney CKD in patients in the therapeutic profile of the early preclinical stage. It is designed for quick calculation of glomerular filtration rate.

Summary:

The aim: to study possibilities of application of telemedical methods using for the organization of the system of distance learning for medical staff training in network of remote health centers with modern methods of providing emergency medical care. The use of telemedicine technology enables application of common standards of modern emergency medical care in a major industrial companies in which health care is based on the outsourcing of medical services. Technical training was implemented using a multi-point video conferencing between the head office and remote enterprises, included in its structure, implemented on the basis of corporate communications. Block-modular training infrastructure has been formed, which included both theoretical components, realized in the form of video lectures, and demonstration part including presentations, video demonstration of techniques of care. The applied technique has been realized over a short period for distant training of more than 300 health care workers providing care to 48 enterprises.

Adherence to recommendations and effectiveness of glucose-lowering treatment in type 2 diabetes patients receiving insulin based in the Nizhny Novgorod region on data of mobile diagnostic and therapeutic centre "Diabetes mellitus"

Summary:

Adherence to treatment is an important factor in diabetes management. Poor adherence may cause ineffective control, especially in insulin-treated patients. Aim of the study: to evaluate the adherence to nutrition, physical activity and self-monitoring in T2DM patients who do and do not receive insulin treatment. Materials and methods. 550 T2DM patients were examined as a part of mobile diabetes center in different areas of the Nizhny Novgorod region. Clinical examination results, HbA1c, creatinine and microalbuminuria were assessed. Adherence to treatment was evaluated by questionnaire Diabetes Self-Care Activities. Results. Insulin-treated patients had longer diabetes duration and worse HbA1 с level and similar levels of adherence related to diet, physical activity, foot care. Only for glucose self-monitoring was adherence higher. Best HbA1c values were observed in patients with adherence in middle region of 3-5 days. Conclusion. Long diabetes duration and worse glycemic control in insulin-treated T2DM patients is not associated with better adherence to treatment. This may be viewed as evidence on insufficient patients education and can be a potential target for improvement of glycemic control.

Summary:

Objective: to study the characteristics of soluble forms of leukocyte membrane molecules and their relationship with disease progression in chronic obstructive pulmonary disease (COPD). Materials and Methods. The study involved 50 patients with COPD. All the patients underwent physical examination, spirometry; serum concentration of antigens sCD54, sCD50, sCD38, sCD25, sCD8, sCD95, sHLA I, sHLA-DR was assessed by ELISA, and the results were compared with serum samples of 30 healthy donors. Results. The level of all soluble forms of membrane molecules of leukocytes decreased in patients with severe COPD. We have found the increased content of the differentiation molecules in patients with moderate COPD, indicating the activation of immune cells during the immune response. Conclusion. There was a correlation between the soluble forms of membrane molecules and impaired pulmonary ventilation, which confirmed the role of immune cells in the pathogenesis of COPD.

Surgical treatment of patients with different forms of jaw deformations complicated by partial loss of teeth

Summary:

The purpose of the work is optimization of diagnostics and planning of surgical treatment of patients with different forms of jaw deformations complicated by partial adentia. Material and methods. Complex treatment has been provided to 16 patients with different forms of jaw deformation complicated by partial adentia. 7 patients out of 16 had the diagnosis of skeletal distal occlusion, nine patients with the diagnosis of skeletal form of mesial occlusion. Results. To estimate the treatment results during 6 months, control orthopantomography, computer tomography and teleroentgenogram have been carried out. Teeth rows have also been photographed. We considered the successful criterion of treatment of normal position of temporomandibular joint from both sides, stable state of central occlusion, the state of chewing apparatus, enabling to fulfill the chewing function completely and harmonic appearance satisfying the patient. Conclusion. Serious achievements have been made in the fields of diagnostics of dentoalveolar anomalies. Opened possibilities of 3D visualizations when planning orthognathic operations together with surgical techniques enable to get prognosing results when treating the patients with different forms of jaw deformations. Progress in this field gives possibilities to a great range of indications for treatment.

Atorvastatin effect on values of endotheli-al condition and acute inflammation in patients on program hemodialysis

Summary:

Objective: To study the effect of atorvastatin on the values of endothelial condition and acute inflammation in patients on program hemodialysis (PHD). Material and methods: The patients were divided into two groups: 28 PHD patients (main group) received atorvastatin, 20 mg once a day for 30 days, and 26 patients (control group) had PHD only. Endothelin 1-21 (ET) level and von Willebrand factor (vWF) activity were determined by enzyme immunoassay, nitric oxide (NO) levels — spectrophotometrically, C-reactive protein (CRP) — by immunoturbidimetric method, fibrinogen — by Clauss method. Results: After 30 days of observation, control patients were found to have NO and ET increase, as well as the reduced activity of vWF relative to initial data, while fibrinogen and CRP levels scarcely changed. The main group patients had similar changes of endothelial condition values, and their fibrinogen and CRP levels decreased. The correlation between the changes of ET and fibrinogen levels was recorded in the patients of the main group. Conclusion: 30-day administration of atorvastatin, 20 mg per day, lowers CRP and fibrinogen levels in PHD patients. It proves the applicability of statins in this group of patients in case of signs of inflammation.

Summary:

The research goal is to estimate a condition of microcirculation in a bladder neck in patients with benign prostatic hyperplasia (BPH) and its value in pathogenesis of bladder neck sclerosis. Materials and methods: Work basis included the analysis of treatment results of 1171 patients with BPH who carried complex examination, operative treatment and further supervision. The basic group was made up of 182 patients with BPH who developed bladder neck sclerosis in the postoperative period. The control group consisted of 200patients with BPH who did not develop the described pathology. clinico-anamnestic, laboratory methods and instrumental studies were used. Results: Disturbance of microcirculation in a bladder neck was considered to be a pathogenetic factor of sclerosis development as one of seri¬ous complications of operative treatment in case of BPH. The presence of microcirculatory disorders was confirmed by histological analysis. Inflammation in the bladder neck should be considered as an aggravating factor. Conclusion: The presence of microcirculatory disorders in the bladder neck in patients with BPH requires preoperative corrective therapy for prevention of sclerosis
