Pharmacopuncture in complex treatment of alcoholic polyneuropathy

Treatment alcoholic polyneuropathy is one of the most important challenges of health care not only in Russia, but throughout the world. Material and methods. A comparison study of efficacy and tolerability ofthefarmacopuncture in 58 patients with alcoholic polyneuropathy [36 (62,1%) male, 22 (37,9%) female, mean age 41,5±12,4 years]. Introduction of thiamin (vitamin B1) in BAP of the cohort studied was accompanied by a positive dynamics of both subjective and objective clinical symptoms. By means of standard ENMG methods the study was made of motor fibrae of median, tibia, fibular and sensory fibrae of median nerves. Results. Farmacopuncture is tolerable and safe. Conclusions. The results of the study confirm an assumption of introduction of thiamin in БАТ efficacy in alcoholic polyneuropathy and allow to recommend the drug for a wide clinical application.

Clinical course, prognosis and prevention of acute postoperative erosive and ulcerative lesions of gastroduodenal zone in older patients

Objective of the research is to improve the results of surgical treatment in patients with postoperative acute bleeding erosions and ulcers of stomach and duodenum by their prediction and validation of methods for prevention and treatment. Materials and methods. A retrospective analysis of 3746 case histories of patients has been made. Acute erosive and ulcerative lesions, complicated by gastroduodenal bleeding, have been diagnosed in 220 cases (5, 9%) in the postoperative period. Older patients have been accounted 62, 8%. Results. Acute erosive and ulcerative lesions have been developed in 5.6% of all operated patients of older age, and 0,3% in patients of middle age. Mortality rate in older patients with bleeding acute erosive and ulcerative lesions has accounted 35,8. Conclusion. The development of gastroduodenal acute erosive and ulcerative lesions is considered to be a risk factor degenerating condition of older patients in the postoperative period. The main areas of acute erosive and ulcerative lesions treatment are antisecretory therapy and endoscopic hemostasis. Endoscopic hemostasis should be accompanied by good antisecretory treatment with antacid effect in the stomach. The use of injections of H2-blockers does not prevent bleeding recurrences

Renal dysfunction and state of metabolic and hemodynamic factors in patients with arterial hypertension

The aim of the investigation is to carry out comparative evaluation of metabolic and hemodynamic indices in patients with arterial hypertension (AH) and renal dysfunction; to study the interrelation between arterial blood pressure level normalization and the presence or lack of microalbuminuria (MAU) in the morning urine portion of patients with AH after therapy with antihypertensive preparations (APs) of various groups. Methods. 121 persons have been investigated, 91 out — patients of both sexes, aged 33-55, with the diagnosis of arterial hypertension of stage II risk III, who have been taking Perindopril, Telmisartan and Bisoprolol for3 months. The control of arterial pressure level, biochemical analysis of metabolic indices and morning urine portion test for microalbuminuria has been carried out. Results. MAU has been revealed in 17,6% patients, occurring more frequently in men than in women. Microalbuminuria is accompanied by reliable decrease of total and ionized calcium and magnesium concentrations, an increase of potassium level in blood plasma, increase of cholesterol, triglycerides, glucose and levels. Patients with AH and renal dysfunction reliably demonstrate higher levels of systolic and diastolic arterial pressure in the morning and evening hours, their normalization effect after APs intake is significantly interconnected with MAU presence. Conclusion. In patients with AH and MAU the main risk factors of cardio-vascular diseases development are more expressed. Microalbuminuria is a risk factor in patients with arterial hypertension and may influence on the basic blood electrolyte balance. While carrying out antihypertensive therapy the presence of MAU should be taken into consideration

Research results of application of reflexo-therapy in correction of myopia

Objective: To develop the optimum scheme of acupuncture, aimed at improving the functional status of the visual analyzer in patients with myopia. Materials. Treatment was conducted in the main group by reflexology courses in 10 procedures and basic therapy in the control group, only basic therapy. The examination included the classic ophthal-mologic research methods, ultrasound and Doppler echoofthalmografy with color mapping of blood vessels — the ophthalmic artery (OA), central retinal artery (CRA) and posterior long ciliary arteries (ZDTSA), the study of visual efficiency and visual assessment of the subjective expression of asthenopia. The results of these studies were subjected to statistical analysis using the method of variation statistics with the computation of the arithmetic mean (M), the average error of the arithmetic mean (m) and the level of significance (p). Differences between mean values were considered significant if the probability of errors on the distribution of Student»s less than 0,05. The result showed a pronounced effect method of acupuncture on the clinical features, hemodynamic and ergonomic eye, the severity of visual fatigue and general condition of patients. Conclusion. The use of corporal-auricular acupuncture technique in patients with myopia allowed to increase the visual acuity without correction in 1,6 times, visual acuity with correction in 1,2 times, lower rates of subjective and objective refraction in an average of 29.8 and 32.4%, increase rates of accommodation of more than 1,5 times, to increase blood flow velocity characteristics in the GA by an average of 9.7, the CAC at 33.4 and ZDTSA by 27,5%. All this is possible to reduce the severity of subjective indicators of asthenopia in 58% of cases and to increase visual efficiency by 11,3%

Dynamics of lower urinary tract of a fetus in intrauterine development retardation

The research goal is to study and analyze the relationship between the level of intrauterine development retardation and urine dynamics indicators of lower urinary tract of a fetus with the help of ultrasonic examination. The urinary function of 89 fetuses was under study. Methods of prenatal ultrasonic cystometry and urofluometry were used to examine the urinary function of a fetus. Statistical software package (Statgraphics 6.0) was used for the statistical analyses of the results. The study has shown that decrease in bladder capacity, increase in residual urine volume and frequent urination are usually the indicators of intrauterine development retardation. The third-degree of intrauterine development retardation is characterized by decrease in diuresis and increase in duration of micturition cycle according to gestational norm

Disturbances in system of cytokines and lipid exchange in patients with psoriasis

Investigation of proinflammatory and antiinflammatory cytokines content in blood serum (IL-1 (3, IL-4, IL-6, IL-8, IL-10, INF-y, IL-17) was carried out for establishment of clinical and diagnostic value of disturbances of immunoregulatory processes and lipid spectrum in patients with psoriasis. Method of solid phase immune-enzyme analysis and indicators of lipid profile on biochemical analyzer Stat Fax were used in the study. It was found out that psoriasis exacerbation was accompanied by change of proinflammatory and antiinflammatory cytokines in blood serum and increase of lipid profile level. The hyperproliferation and inflammatory skin changes in case of psoriasis exacerbation were accompanied by hyperproduction of cytokines and disturbance of immunoregulatory processes to Th-2 cellular answer including autoimmune processes. The described changes led to raised synthesis of lipids and to their entry into peripheral blood flow. Determination of proinflammatory cytokines and lipids level in blood serum in patients with psoriasis can be used for an estimation of severity of disease, its development prognosis and administered therapy
