Clinical and immunological parallels in the diagnosis of primary open-angle glaucoma.

Summary:

The purpose of study is an assessment of the informative value of various diagnostic parameters in monitoring and prognosis of the state of the visual organ in patients with primary open-angle glaucoma. Material and methods. 216 patients (216 eyes) were examined. 126 patients (126 eyes) with the diagnosis of primary open-angle glaucoma I, II or III stage were included in the main group (I), and 90 patients (90 eyes) without ophthalmologic pathology were included in the control group (II). All patients underwent complex examination. Laser confocal scanning tomography for the analysis of the optic disc morphology was performed using a Heidelberg Retina Tomograph II from Heidelberg Engineering (Germany). Evaluation of the field of view was carried out using the computer static perimeter Oculus twinfield-2 (Germany). Electrophysiological studies (apparatus Roland Consult, Germany) included: the study of visual cortical potentials on the outbreak, the pattern-electroretinogram, photopic negative response. Neurotrophic brain factor, ciliary neurotrophic growth factor and protein NF-200, S-100 protein, monocyte chemoattractant, vascular endothelial growth factor were determined in the serum. Results. An increase in the concentration of the S-100 protein with progression of POAG testifies to the existence of parallels between apoptosis of retinal ganglion cells and systemic neurodegenera-tion affecting the central nervous system as a whole. The importance of electrophysiological research methods, which give information about the third neuron of the visual pathway, has been confirmed. Pattern-electroretinogram, photopic negative response most reflect abnormalities in the work of ganglion cells of the retina, including before the appearance of structural changes in the optic disc and the retina. Conclusion. The interrelation of dynamics of functional, morphological and immunological criteria of progression of POAG is revealed, which allows to predict the further course of the pathological process. Since the study involved patients with IOP target values, it is important not only to maintain IOP on target values, but also to carry out neuroprotection.

Induction and reparation of double-strand DNA breaks in V79 cells continuously exposed to low dose-rate Y-radiation

Summary:

Aim: to study the patterns of changes in the number of DNA double-strand breaks (DSB) in mammalian cells continuously exposed to low dose-rate y- radiation. Material and methods. Chinese hamster lung fibroblasts (V79) were used in this study. The y- irradiation of cells at a dose rate of 0.1 mGy/min was performed using the «Gamma-Panorama» unit (Cs-137). The fluorescence immunoassay of the phosphorylated H2AX-histone (y-H2AX) foci was used to investigate the DNA DSBs formation. Frequency of apoptotic cells was evaluated using «DNA halo» assay. 5 (6) — chloromethyl-2,7-dichlorodihydrofluorescein diacetate was used to estimate the reactive oxygen species (ROS) production. Results, it was showed that continuous low dose-rate irradiation of Chinese hamster V79 cells induces an increase of the y-H2AX foci number and ROS production rate at the early stages of exposure time (6-24 h, doses 3.6-14.4 cGy), while increasing exposition time and, therefore, the radiation dose (48-72 h, 28.8-43.2 cGy) caused a decrease in these endpoints to almost the control level. There was observed no significant changes in the frequency of apoptotic cells. Conclusion. It is assumed that the processes causing the DSB amount changes in mammalian cells continuously exposed to low dose-rate y-radiation are associated with the development of oxidative stress and subsequent activation of cellular antioxidant defense systems.

Summary:

The aim of the research was the study of the cell proliferation indices of the gum epithelial cells in patients with parodentium inflammatory diseases accompanied by gastroesophageal reflux disease (GERD). Material and methods. 160 patients were examined, 60 patients with parodentium inflammatory diseases accompanied by NEGERD, 40 patients with parodentium inflammatory diseases accompanied by GERD. The comparison groups consisted of 40 patients with diseases of parodentium without esophagus pathology and 20 healthy persons. Results. On the basis of the conducted clinical-endoscopic, immune-histological and morphological examinations it was established that gingivitis and periodontitis are successive stages of disturbance of the cellular homeostasis of gum epithelial cells. Conclusion. The enhancement of the processes of proliferation and apoptosis promotes the progression of the structural alterations in the tissues of parodentium and oesophagus and may serve as additional diagnostic and prognostic criteria of the diseases of parodentium accompanied by GERD.

Summary:

The research goal is to determine clinical, endoscopic and morphological signs of chronic duodenitis in children. Materials and methods: The diagnostic value of molecular markers has been revealed by immunohistochemical research of biopsy. It has been received from endoscopic examination of 32 children aged from 3 to 17 years old with chronic duodenitis. Morphometric investigation of markers expression has been carried out by means of analyzing system of digital images of Mikrovizor medical uVizo-103. Results: The index of proliferation has been higher in cells of cover epithelium than in glands. The inductor expression of apoptosis Bax has been poorly expressed in both groups of patients. It has been proved that proliferative processes are predominant in this pathology. Conclusion: It has been found out that proliferation of significant activity in the cover epithelium accompanies the development of preatrophic processes in children with chronic duodenitis. The greater degree of regeneration has been marked in crypts.

Apoptosis and premature aging in patients with metabolic syndrome

Summary:

The Research Objective: Correlation of apoptosis and senilism in patients with metabolic syndrome has been studied. Materials. Apoptosis inductor protein p53 concentration, insulin level in blood serum, biological age, ageing rate coefficient of 270 patients with metabolic syndrome of the age from 30 till 60 have been determined. Among them there have been 162 men and 108 women. Ageing rate coefficient and biological age have been calculated by A. G. Gorel-kin's and Pinchasov's formulae, protein p53 concentration and insulin in blood serum have been determined by enzyme multiplied immunoassay method. Results. Protein p53 concentration of the patients with metabolic syndrome has been increased and worked out 1,59 (1,361,91) U/ml, biological age, ageing rate coefficient have been increased — biological age to 6 years, ageing rate coefficient has been 1,32. Conclusion. These patients have been referred to the group with high risk of early pathology development because the processes of senilism are proleptic. The direct connection between protein p 53 and ageing rate coefficient (r+0,63, p< 0,05) and between apoptosis marker protein p53 and biological age (r+0,6, p< 0,05) have been determined.

Regularities of cytokine status changes in chronic lymphocytic leukaemia and their role in pathogenesis of progressive forms of disease

Summary:

The aim of the research is to study and to analyze results of investigations of cytokine blood profile in patients with B-cellular chronic lymphocytic leukaemia. Materials and methods: Complex investigation of 60 patients with B-cellular chronic lymphocytic leukaemia has been carried out. Patients have been randomized into 4 groups of observation according to the severety of disease (classification of Rai K. R., 1975). Accepted methods of somatic status and he-matologic indices as well as method of flow cytometry for establishment of B-lymphocytes immune phenotype have been used. Cytokine levels were detected by enzyme immunodetection. Results: Regular peculiarity of cytokine status change in different forms of chronic lymphocytic leukaemia development has been an increase of interleukine-4, interleukine-6, interleukine-7, interleukine-10 and tumor necrosis factor-alpha levels in serum that is one of the main pathogenetic factor of intercellular interaction abnormality in lymphoid tissue in the chronic lymphocytic leukaemia development. Conclusion: Increased indications of cytokines content in blood may be used as diagnostic criteria of pathology severity and in evaluation of effectiveness of complex therapy of the disease, as well as a classification sign of severity of chronic lymphocytic leukaemia

The research work presents an analysis of literature review and results of investigations on the problems of etiology, pathogenesis, classification and peculiarities of clinical manifestations of chronic lymphocytic leukaemia. Special attention is paid to both contemporary conceptions of carcinogenesis, reflecting the role of proto-oncogenes activation, an-tioncogenes and apoptosis genes suppression in mechanisms of neoplasia development and «specific mechanisms» of oncogenic transformation induction of B-lymphocytes and the subsequent development of the stages of promotion, progression and marked clinical manifestations of formation. This research is dedicated to the problem of choosing treatment tactics in catarrhal form of acute appendicitis. The results of investigation and treatment of 1468 patients with acute appendicitis of our region were analyzed. Large specific gravity (26%) of patients who had been operated with regard to acute appendicitis was exposed. With the purpose of improvement of surgical treatment results of patients with acute appendicitis we have worked out and instilled a diagnostic system, which is a totality of invasive and uninvasive methods of investigation that allows to divide clinical material into two categories: one of them is with existence of inflamed process and the other is without it.
