Surgical treatment of isolated aseptic acetabular loosening of the hip joint

Summary:

Aseptic acetabular loosening is one of the most common complications of primary hip arthroplasty Its treatment is possible in the volume of replacement of only the endoprosthesis cup with the preservation of a stable correctly oriented femoral component. The sequence of actions consists of provision of access to the acetabulum component and its replacement, after which the necessary offset is selected by changing the size of the head and installing a new pair of friction. Currently, the vast majority of the installed heads during the primary and revision hip replacement have a standard size forthe cone of the femoral component 12/14mm. According to the observations, in 3% of cases, there are femoral components with non-standard cone sizes 11/13 mm, usually established 7-8 years ago. The absence of heads forthe necessary cone leads to the need to replace the stable correctly oriented femoral component, which is accompanied by an increase in the time of intervention, the volume of intraoperative blood loss and the risk of intraopera-tive peri-prosthetic fracture. We represent the clinical case of a 75-year-old patient with a dislocation of the acetabular component and a stable correctly oriented femoral component, which during the audit intervention due to the lack of heads for an irregular cone size, total replacement of the components was made.

Summary:

Diaphyseal fractures of the femur are rather common. In the treatment of fractures of the femur various methods of osteosynthesis can be used: intramedullary, of plate, transosseus. In our country external fixation using external fixation devices has traditionally been popular. Objective. The analysis of the frequency of use of transosseous osteosynthesis in the treatment of fractures of the femur and the identification of the most common indications for the use of the method in the present practice. Materials and Methods. The analysis of the structure of external fixation in fractures of the femur, on the experience of treatment of 126 patients with diaphyseal fractures of the femur. Results. The results of the study revealed that the structure of the osteosynthesis of fractures of the femur transosseous osteosynthesis with the use of external fixation devices is not considered to be common and is used as an alternative method of treatment. Conclusion. The treatment of fractures of the femur using transosseous osteosynthesis is currently used mostly for open, comminuted fractures and revision osteosynthesis, as well as a high risk of infectious complications in the zone of fracture, due to complexity of the method, the lowest strength of fixation and a relatively high percentage of non-specific complications.

The Pupose: to analyze X-ray, anatomical and biomechanical features in patients with developmental hip dislocation. Material and Methods. The study included 73 patients (57 women and 16 men) with the developmental hip dislocation at the age of 31 to 76 years. The clinical method, X-ray, gait analysis and visual analogue scale VAS, McGill inquiry. Results. The comparative analysis of X-ray, anatomic and biomechanical features in patients with different severity of developmental hip dislocation. Conclusion. Features of the functional status of patients with different severity of developmental hip dislocation enable to predict the tactics of surgical treatment and individual rehabilitation program.
