The use of clinical analysis of movements in evaluation of motor functional status of patients after total hip replacement

Summary:

Aim: to estimate functional status of coxarthrosis in patients requiring total hip replacement of the two hip joints. Material and methods. The biomechanical examination of 94 patients with bilateral primary coxarthrosis before and after total hip replacement was performed using clinical stabilometric software complex. The ability to perceive the mechanical load during standing and walking was evaluated at different stages of the treatment. The difference between the samples was estimated with the use of Mann — Whitney U-test. The rank correlation of biomechanical parameters was measured by Spearman coefficient. Results. It was revealed that the most responsive indicators are the transfer period, the first and second periods of double support and the deviation of the center of pressure relative to the average position in the frontal plane. Conclusion. After surgical treatment there was observed some improvement: the-left-and-right-step asymmetry decrease, rhythm rate increase, improved ability to maintain body balance, jog reactions increase.

In the course of the study the foreign and native publications concerning total hip replacement have been analyzed. The data from National Registers of various countries are presented in the study. They highlight some issues upon total hip replacement, technologies of surgical intervention advancing and most important risk factors for endoprosthetic ball dislocation.

Morphometric justification of algorithm of the choice of method of surgical treatment of dysplastic coxarthrosis

Summary:

Purpose: to define regularities of variability of bone structures of a hip joint depending on type of dysplastic coxarthrosis and to develop tactics of total endoprosthesis replacement. Objects of research. 106 patients with dysplastic coxarthrosis of varying severity, passing treatment on the basis of Federal State Budget Institution of Saratov Scientific Research Institute of Traumatology and Orthopedics from 2005 to 2014 applying methods X, KT-, MRT-metry variation and statistical methods. Results: According to anatomic features of a hip joint at dysplastic coxarthrosis of various types algorithms of choice of acetabularand femoral components of endoprosthesis, and also tactics of total endoprosthesis replacement in groups of research are developed. Conclusion. The choice of a standard size of an endoprosthesis of a hip joint needs to be carried out taking into account X-ray anatomic features of a hip joint and degree of expressiveness of the dysplastic changes. At dysplastic coxarthrosis of the l-ll types it is necessary to carry out standard total endoprosthesis replacement; at coxarthrosis of the III type —total endoprosthesis replacement in combination with plasticity of a roof of acetabular hollow; at coxarthrosis of the IV type — total endoprosthesis replacement in a combination from the shortening femur osteotomy.

Summary:

The Pupose: to analyze X-ray, anatomical and biomechanical features in patients with developmental hip dislocation. Material and Methods. The study included 73 patients (57 women and 16 men) with the developmental hip dislocation at the age of 31 to 76 years. The clinical method, X-ray, gait analysis and visual analogue scale VAS, McGill inquiry. Results. The comparative analysis of X-ray, anatomic and biomechanical features in patients with different severity of developmental hip dislocation. Conclusion. Features of the functional status of patients with different severity of developmental hip dislocation enable to predict the tactics of surgical treatment and individual rehabilitation program.
