Antiangiogenic properties of beta-blockers

Summary:

 Objective: determination of comparative antiangiogenic effectiveness and influence of beta-blockers on the level of a vascular endothelial factor of growth in an experiment. Material and Methods. For determination of antiangiogenic effect of beta-blockers, comparative studying of their influence on the level of a vascular endothelial factor of growth in an experiment on 72 nonlinear laboratory rats, by average weight 180±20 g which were conditionally divided into 6 groups is executed: 1st — control, 2nd — negative control (experimental ischemia (El), crossing of femoral vessels), 3rd — positive control (El with bevacizumab introduction), 4th — El with introduction of a timolol, 5th — El with introduction of a betaksolol, 6th — El with introduction of interferon alpha 2b. Results. The VEGF levels were: in 1st group 1.5±0.3 pg/ ml, in 2nd group 20,3±3,2 pg/ml, in 3rd group5,8±0,9 pg/ml, in 4th group 13,8±1,4 pg/ml, in 5th group 192,0±2,3 pg/ml, in 6th group 11,1 ±2,2 pg/ml. Results of microscopy and immunohistochemical research demonstrate lack of activation of processes of neoangiogenesis in 1 st group of animals. At animals of 2nd and 5th groups along with the expressed inflammatory processes the neoangiogenesis phenomena are established. Conclusion: Beta-blockers show the direct or mediated negative impact on synthesis of VEGF and oppression of neoangiogenesis. The activity of selective beta-blocker concerning neoangiogenesis suppression was lower in comparison by activity non-selective that allows to consider the timolol effective antiangiogenic remedy.

Regional peculiarities of the morbidity of psoriasis and psoriatic arthritis

Summary:

Objective: to analyze the incidence of various forms of psoriasis in the population of the Saratov region for the period from 2009 to 2016 Material and Methods. We studied the indicators of the total and primary incidence of psoriasis in the adult population in the Saratov region for the period from 2009 to 2016 in comparison with similar indicators for the Russian Federation (RF), according to the Ministry of Health of the RF Results. It is revealed that for the period 2009-2016 the overall incidence of adult psoriasis in the Saratov region increased by 5.8%, in the RF by 13.8%; the overall incidence of adult psoriatic arthritis in the Saratov region increased by 8.1 %, in the RF by 2.2%; Primary morbidity of the adult population of psoriatic arthritis in the Saratov region increased by 11.4%, in the RF by 2.3%. Primary incidence of psoriasis for the period 2009-2016 among the adult population in the Saratov region decreased by 18.5%, in the RF by 45%, which also reflects the dynamics in the RF. Conclusion. The steady increase in the overall incidence of psoriasis and psoriatic arthritis in the adult population, the level of primary morbidity in psoriatic arthritis directly affects the costs and level of the state budget and requires ways to find ways to improve the organization of medical care of a dermatovenerological profile.

Modern aspects of progression of onychomycoses of feet in the Krasnodar region

Aim: to study clinical epidemic-logic peculiarities of progression of onychomycosis of feet in the Krasnodar region for the period from 2015 to 2017 Material and Methods: cultural study of subunguial content of 56 patients ill with onycho-mycosis of feet, analysis of annual reports of the State Clinical Dermatovenerologic Dispensary of the Ministry of Public Health of the Krasnodar region for the last 3 years, 298 out-patient medical records, questionnaire of 68 patients with the given disease. Results. An increase in the treatment of patients with onychomycosis in the polyclinic department of the dispensary was noted. The main causative agents are Trichophyton dermatophytes. A nail plate is destroyed more often by the normotrophic type from a distal lateral border. People over 50 are more susceptible to the disease. They request medical assistance mainly in case of progression of profound clinical forms of onychomycosis. Cosmetic defect of the nail is the main noted motif for visiting a doctor. Considerable part of patients, who had undergone the questionnaire, did not worry about possibility to infect other people. In most cases wrong shoes and nonobservance of personal hygiene caused progression of onychomycosis. Dermatomycosis and chronic allergodermathosis were registered more often from the co-morbidity More than 1/3 of patients had been taking antimycotic medications on their own initiative before visiting a doctor. Conclusion. Obtained data allow characterizing a modern patient with onychomycosis as an unapparent circulator of fungal infection. The findings of the study can help a practitioner in development of the most effective preventive measures and choice of optimal therapeutic tactics.

Some epidemiological aspects of syphilis infection in the Saratov region (2007-2017)

Summary:

Aims: to study the epidemiological aspects of syphilitic infection and neurosyphilis in the Saratov region for the period 2007-2017. Material and Methods. On the basis of statistical materials of the state statistical observation for 2007-2017 the indicators of syphilis incidence are analyzed. Results. The analysis of the data obtained from the state statistical supervision forms during 2007-2017 has shown the decreasing tendency of number of newly reported cases of syphilitic infection. The indicators of morbidity caused by late syphilis and neurosyphilis have increased. The number of regional cases of syphilitic infection with involvement in the pathological process of the nervous system exceeds similar indicators in Russia, and late asymptomatic neurosyphilis prevails among clinical forms. Conclusion. In the Saratov region for the period 2007-2017 there is a decrease in the incidence of syphilis, which is accompanied by an increase in hidden forms of infection and neurosyphilis.

Readiness to realize the skills of professional communication among senior students of a medical university

Summary:

Aim: to assess the readiness for effective communication of senior students of medical university. Material and Methods. A survey of 158 students of the 5th and 6th courses of the medical faculty of SSMU was conducted in February 2017 according to the original questionnaire aimed at assessing the theoretical knowledge in the field of communication and readiness to communicate with patients in practice. Results. The study revealed a number of problematic issues related to the communicative competence of senior students of the medical University. Most respondents (88%) rated their training in communication technologies as high and sufficient, and knowledge of theoretical issues does not always meet the requirements of today. Despite the support of the majority of students for a paternalistic approach to the relationship between the doctor and the patient (44% in general, 94,7% among sixth- year students), many respondents (85%) agreed with the involvement of the patient in the process of discussing the planned diagnostic and therapeutic measures, more than half of the students (57,6%) expressed a desire to undergo additional training on the basics of communication. Conclusion. In accordance with the new challenges in the native health care training of graduates of medical universities should include the formation and improvement of communicative competence with the realization that in the modern system of relations the Central person is a patient with his problems and

Medical and organizational issues of patients' preoperative fears management (analytical review)

Summary:

 The article deals with the problem of preoperative patient fears. Specific characteristics of preoperative fears and the mechanisms of their occurrence were described. According to the scientific literature analysis the typology of preoperative fears was performed. Within the framework of the author's approach the directions of the following theoretic understanding and practical solutions of indicated problem were presented. They included health managers focusing on interpersonal communication of patient and medical specialist; further analysis of factor influencing interpersonal communication of patient and medical specialist under disease condition requiring surgery; development of psychologically based medical care and postoperative rehabilitation quality indicators.

Summary:

The purpose of scientific work was to substantiate the relevance ofthe problem of transplant tourism, which is a way for a recipient to obtain an organ for transplantation bypassing national wait lists by finding a donor in a foreign country on a commercial basis. The authors have performed analysis of literary sources related to issues ofthe development of transplant tourism around the world, origins of its existence as well as possible ways to prevent the following practive. In conclusion, authors underline the necessity to establish the development of transplantology based on principles of gracious organ donations and fairness in terms of recipient election.
