Оценка роли микроорганизмов респираторного тракта у больных с осложненным течением острых респираторных вирусных инфекций

Резюме:

Изучена литература за 1987-2012 гг. Показано, что широкое распространение острых респираторных вирусных инфекций (ОРВИ) и частота развития при них осложнении имеют большое социально-экономическое значение во всем мире. Поэтому выявление этиологических и сопутствующих факторов развития ОРВИ является актуальной задачей практического здравоохранения. Большое значение имеет не только определение вида возбудителя, но и наличие у него факторов патогенности. Чаще всего этиологическим агентом бактериальных осложнений ОРВИ становится Moraxella catarrhalis, отличающаяся большим набором маркеров вирулентности. В развитии патологического процесса в респираторном тракте важную роль играют адгезины, ряд ферментов и токсинов, продуцируемых М.catarrhalis и позволяющих ей размножаться и персистировать в организме больного. Большую опасность представляют штаммы М.catarrhalis, устойчивые к (3-лактамным антибиотикам а также характеризующиеся мультирезистентностью, что значительно снижает результативность проводимой терапии. Отсутствие протокола лабораторных исследований при острых воспалительных заболеваниях респираторного тракта приводит к снижению уровня этиологической расшифровки этих заболеваний, неадекватному этио-тропному лечению и, как следствие, к развитию осложнений и снижению эффективности проводимой терапии. Следовательно, необходимо совершенствовать систему микробиологической диагностики при ОРВИ и тактику ведения таких больных.
